Objective : To establish an ideal and reversible animal model of partial obstruction of ureter; observing renal function lesion degree in different periods,renal function restoration extent after removing obstruction and pathological changes before and after disentangling obstruction. To investigate the effect on TCM of promoting circulation of qi and the blood treating obstructive nephropathy. Meanwhile, approaching TCM of supplementing vital energy and promoting blood flow protecting renal function of obstructive kidney and pathological improved condition .Methods: 133 male Wistar rats were divided into 4 groups. There were 48 rats in the group of ureter-psoas major on opposite side ventrofixation (A group ) , 40 in the group of renal duct incomplete ligature (B group), 40 in the group of ureter-psoas major on the same side ventrofixation (C group) , and 5 in the blank control group (D) .The animal model of partial obstruction of ureter was set up using ureter-psoas major on opposite side ventrofixation,renal duct incomplete ligature and ureter-psoas major on the same side ventrofixation respectively. The survival rate and model achievement ratio were evaluated after operation. The renal function of obstructive kidney and total renal function of rats in A group are determined by means of isotope nephro-dynamic scan later in 6 different phase respectively (1,2,4,6,8,10 weeks after operation) .The renal pathological changes are also observed in different period as well. These indexes above are done to rats in B group, too. 48 male Wistar rats were randomly divided into pathologic group, control group and TCM group, 16 each group. The animal model of partial obstruction of ureter was set up with ureter-psoas major on opposite side ventrofixation. The renal function of obstructive kidney and total renal function were determined by means of nephro-dynamic scanning 3 and 6 weeks after operation. At the same time, the renal pathological changes were observed in different phases.Results: 1.The model achievement ratio were 97.9%, 37.5% and 60% in group A, B and C group respectively. The model achievement ratio in A group was higher than that of B or C group. The survival rate were 97.9%,67.5% and 87.5% in group A, B and C group respectively and the rate of A group was higher than that of B and C as well . It was found that high GFR of A group occurred 1 week after obstruction. Apart from, GFR of the left kidney after obstruction was lower than that of before obstruction in each group. Of them, GFR in A group decreased at slow speed , B decreased at faster rate and C decreased at the fastest speed. If the obstruction was disengaged in 4 weeks, the renal function and fibrosis were improved obviously. However, the renal function and pathological change didn't restore to the normal condition if the obstruction is disengaged in 6 weeks. If the obstruction was disengaged 4 weeks later, the renal function of post-treatment made obvious improvement compared with before obstruction. There was much difference between the two groups (P<0.01) .However, the renal function could not restore to the normal condition if the obstruction was disengaged in 6 weeks. The renal function of B group could not restore to the normal if the obstruction was disengaged in 2 & 4 weeks. The renal function of C group could restore to some extent if the obstruction was disengaged in 2 & 4 weeks. After obstruction was disengaged 4 & 6 weeks later, the total GFR in A group was much better than that before obstruction is disengaged(P<0.01) . The total GFR in B & C group had no difference before and after obstruction is disengaged 2 & 4 weeks later, which was less than the normal level (P>0.05) . The nephrogram took on obviously obstructive curve after obstruction, c section delayed markedly, visualization of kidney turned light and the image of obstructive kidney turned from light to thick. After obstruction was disengaged 4 weeks later, the renal function of the left kidney in A group recovered to the normal and obstructive curve disappeared. However, after obstruction was disengaged 6 weeks later, the obstructive curve remained .It was found that the obstructive curve in B & C group made no difference compared with before disengaging obstruction.2.The renal pathology of the rats in the pathologic group was showed in the following under microscope 3 weeks after obstruction, : slight fibrosis, Inflammatory cell infiltration of renal interstitium, widening of renal interstitium, expansion of proximal convoluted tubule and distal convoluted tubule, occasional atrophie of renal tubule and no pathological changes on renal glomeruli.6 weeks after obstruction, it was discovered that the degree of the renal fibrous degeneration was aggravated, cortex of kidney was thinner than normal and the damage index of on interstitium of renal tubule was increased .Compared with the pathologic group, the renal pathologic condition of the control and TCM group was better: inflammatory cell infiltration degree of renal interstitium abated, breadth of renal interstitium narrowed, tubular ectasia improved and the damage index of on interstitium of renal tubule lowered. After obstruction, GFR of the left renal in the pathologic group decreased quickly. In contrast, GFR of the left renal in the control and TCM group decreased slowly. There was significant difference among the 3 groups. And there was no difference between TCM group and the control group. Nephrogram: nephrogram of the 3 groups showed obviously obstructive curve, c section delayed significant, the visualization of kidney turned light.Conclusions: 1.According to ureteral angulation theory ,it is a scientific and feasible to establish animal model of partial obstruction of ureter with ureter-psoas major on opposite side ventrofixation. The method is simple and exercisable. The model achievement ratio is high. The model is made in 2 weeks after operation and the obstruction is reversible. The renal lesions make a slow advancement after obstruction. With time went on, the renal functional lesion becomes more serious, the renal morphology changes obviously. Interstitial fibrosis of the kidney is found in 4 weeks after obstruction. If the obstruction is disengaged in 4 weeks, the renal function and fibrosis are improved obviously. However, the renal function and pathological changes do not restore to the normal condition if the obstruction is disengaged in 6 weeks. At the same time, it is an unstable to set up animal model of partial obstruction of ureter with ureter-psoas major on the same side ventrofixation and renal duct incomplete ligature. The 2 model achievement rate is low.2.Obstructive nephropathy can be diagnosed in the early stage by means of nephro-dynamic imaging, which is sensitive to judge functional lesion degree in early phase.3.The principal of promoting circulation of qi and the blood is an effective in the treatment of obstructive nephropathy. The TCM of supplementing vital energy and promoting blood circulation can improve interstitial fibrosis of the kidney and protect renal function.
