ObjectiveThe incidence rate of prostate cancer is notable increasing in China. PSA is the most important tumor marker to diagnose this cancer,it only have tissue specificity,no tumor specificity,which have higher false positive in detection, furthermore, the patients can not exclusion from prostate cancer when PSA is in the normal range.DD3 gene is high expression in human prostate cancer tissue and metabasis necrosis focus. The objective of our research is justify DD3 whether could displace PSA in early diagnosis and prognostic judgement of prostate cancer.Materials and MethodsThe urine sample were collected,including non-prostate disease, benign prostatic hyperplasia,and prostate cancer patients;the total RNA have extracted for RT-polymerase chain reaction; agarose gel electrophoresis were applied to confirm PCR product of DD3 usingβ-actin as house-keeping gene, the semi-determinalion of DD3mRNA were carry out simultaneously. Fluorescent quantitation polymerase chain reaction were applied to relative quantitative determination of DD3mRNA; eventually, SPSS 13.0 were used for data analysis.Result1 DD3 gray scale ratios have no difference between non-prostate disease and benign prostatic hyperplasia patients,but it have difference between non-prostate disease and prostate cancer patients. The relative quantitation value of DD3 have difference between these three kinds of patients; The relative quantitation value of DD3 of prostate cancer patients is 14.1 times of benign prostatic hyperplasia patients,24.1 times of non-prostate disease patients.2 The relative quantitation value of DD3 have difference between three serum PSA grade. The benign prostatic hyperplasia and prostate cancer patients which serum total PSA located in 4-10 ng/ml, tPSA fPSA DD3 gray scale ratios have no difference,but the relative quantitation value of DD3 have significant difference.3 DD3 gray scale ratios and relative quantitation value of DD3 have significant difference between every Gleason score grade, they become higher when differentiation degree is higher. DD3 gray scale ratios and relative quantitation value of DD3 are significance positive correlation with Gleason score;DD3 gray scale ratios is positive correlation with prostate volume,but not obviously.4 Applied receive operative characteristic curve analysis data,the area under the curve of tPSA,fPSA, DD3 gray scale ratios,relative quantitation value of DD3 is 0.788,0.744,0.921,0.942, respectively. The efficacy of relative quantitation value of DD3 is maximum, DD3 gray scale ratios is the next in order.DiscussionA lot of studies indicate that the false positive rate of PSA is 25%, the false negative rate is 38%-48% when PSA in 0-4ng/ml; in addition,several multicentre clinical trials shown that rectal touch could find 10%-21% prostate cancer patients which PSA was normal. In the patients which PSA>4.0ng/ml, the biopsy can only diagnose 25%- 30% prostate cancer patients.de Kok et al found,the content of DD3 in prostatic carcinoma tissue was 34 times than innocence tissue,even if the content of prostate malignant cell was less(<10%), DD3 could expresss distinctly and exactly. Clinically, RISH method,RT-PCR technique can be applied in the prostate nyxis cytology biopsy and body fluid (blood,sperm,urine, prostatic massage emulsoil ) detection. To contrast,DD3 detection in urine can be accepted by patients,the urine sample can easier obtain.The relative quantitation value of DD3 of prostate cancer patients is 14.1 times of benign prostatic hyperplasia patients,24.1 times of non-prostate disease patients,the difference of it is more enormous than serum PSA. Applied receive operative characteristic curve analysis data,the area under the curve of relative quantitation value of DD3 is biggest(0.942),the detection efficacy of relative quantitation value of DD3 is maximum when synthetic consider sensitivity and specificity. Gleason score was universal patho-grade in western countries, Gleason score could predict prostate cancer patients' prognosis. This research fould that DD3 gray scale ratios and relative quantitation value of DD3 have significant difference between every Gleason score grade, they were significance positive correlation with Gleason score. So we conclude that the quantitative determination of DD3 relative quantitation value should guide prostate cancer patients' prognostic. The relative quantitation value of DD3 have difference between three serum PSA grade. The benign prostatic hyperplasia and prostate cancer patients which serum total PSA located in 4-10 ng/ml, tPSAs F/T DD3 gray scale ratios have no difference,but the relative quantitation value of DD3 have significant difference. These results shown that the detection of DD3 relative quantitation value could produce a marked effect in the differential diagnosis of benign prostatic hyperplasia and prostate cancer.The molecular biology characteristic and function of DD3mRNA was still not completely understsnd; The detection methods was not standardizely , critical value was not definite when DD3mRNA applied to screening and earlier differential diagnosis. How to raise the capability of judge turnover when the detection of DD3 correlate with prostate cancer clinical stage, patho-grade. All of these wait for the deep research in future on account of large scale people group. ConclusionThe quantitation detection of DD3mRNA in urine could be applied to the screen of prostate cancer; the diagnosis capability of it was better than tPSA and F/T when they used to discriminate prostate cancer and benign prostatic hyperplasia; eventually,it was a useful tumor marker which could guide the prognostic of prostate cancer.
