Objective:To investigate the predictive value of heart rate turbulence(HRT)and heart rate variability(HRV)in patients(pts)with essential hypertension(EH)complicated by premature ventricualr beat,by nonsustained ventricular tachycardia(NSVT).Methods:Choose EH complicated by NSVT 75 of pts,and select by EH complicated by abiogenesis premature ventricualr beat(VE)116 of pts,EH complicated by frequent VE 93 of pts, at the same time,52 health persons are collected as control.All persons are monitored with Holter in 24 hours,and the Holter instrument has the function software of HRT,HRV,average heart rate,QT variation analysis.Patients are classified into the following HRT categories: HRT0(both TO and TS are normal);HRT1(either TO or TS are abnormal);HRT2(both TO and TS are abnormal).Result:①The TS and the HRVI of the occasional group and the frequent group and the NSVT group are significantly lower than those of the control group,and the TO is significantly higher(p<0.01).Compared with occasional group,the TS of the frequent group and the NSVT group obviously increase(p<0.05).However,there is no significantly difference in TO,TS, HRVI between the frequent group and the NSVT group(p>0.05).②The LF,HF of the occasional group,the frequent group and the NSVT group decrease significantly as compared with contral group(p<0.01)③The positive rates of TO,TS,HRVI in the occasional group,the frequent group and the NSVT group are obviously higher than those in the control group(p<0.01),moreover in the frequent group and the NSVT group the positive rates of TS are higher than those in the control group(p<0.01).The positive rates of TO and TS there are no statistically difference between the frequent group and NSVT group.(p>0.05)④The HRT ratings manifest:0 classes:The control group is maximal,occasional group is secondary, frequent group and NSVT group is minimal.One class:The occasional group,the frequent group, the NSVT group are all higher than the control group.2 classes:The frequent group and the NSVT group are maximal,occasional group is secondary,the control group is zero.⑤The QT extension of NSVT pts and VE pts are significantly higher than the control group.(p<0.05).⑥Sensitivity of TO,TS,as well as specificity,positive predictive value,negative predictive value are all higher than HRVI.Sensitivity and specificity of TS is significantly higher than those of TO.Conclusion:HRT and HRV are both very useful for risk prediction in pts with EH complicated by premature ventricualr beat,by NSVT.Among them,the HRT is a stronger than HRV,more accurate,more systematical index.The predictive value of TS is larger than TO,and positive predictive accuracy is higher.
