Essential hypertension (EH) is a disease characterized by systematic blood pressure increasing. As a systematic disease, EH can impair not one but many target organs, such as heart, brain, kidney and so on.In the last two decades, the significant correlation between autonomic neuropathy (ANP)and cardiac mortality, especial sudden cardiac death was recognized, Early checking and prediting risk was paid.more attention to autonomic neuropaphy patients in EH. HRV is a non -hurt index in evaluating ANP activity. So, HRV is a ration index in appraising sympathetic and parasympathetic activity. It's also indirect index in reflecting ANP activity.In this study, one is to investigate the characteristics of HRV in EH patients and the relationship with boold pressure increase. To evaluate the value of HRV in diagnosing ANP in EH patients.The other is to investigate the characteristics of HRV in EH patients who with or without ventricular hypertrophy (LVH), and if they were combinated with ventricular arrhythmia(VA). Through this, we can know more about its prognosis of HRV. and decrease the rate of sudden cardiac Death. HRV will be used in the therapy of EH to reduce the risk of sudden cardiac death.Method 1. Subject(1) EH group : 60 hypertensive patients were selected, 34 patients were male, 25 patients were female,Their average age was(55±10) years old ( from 34 to 70) .They are all accordance to the standard of 2004 WHO/ISH in EH, 20 cases in grade-1 EH group, 22 cases in the grade-2 EH group and 18 cases in the grade-3 EH group . It is also be devided into two groups. 34 of them without left ventricular hypertrophy(LVH) , 26 with LVH.(2) Control group: there were 40 health examination people selected,26 patients were male, 14 patients were female, their average age was 51.5±8.0year.The two groups have no sex- and ago-matched. Their have no significantly statistical differences (P<0.01)2. MethodBy 24-hour dynamic electrocardiogram(Holter) and related software system , we detected four indices of time domain of HRV, the standard deviation of all NN intervals (SDNN), the standard deviation of the average of NN intervals in all 5-minutes of the entire recording(SDANN), the root mean of square of difference between adjacent NN intervals(rMSSD). T-test was used between the two groups P<0.05 showed significantly statistial difference.ResultsThere are no significant difference in the characteristics of the patients, such as age, The male/female ratio , heavy, Height, Smoking percentage, the mean time of Holter monitoring and so on. 1. Compare with the control group all indeces of HRV were markedlyand significantly reduced in EH patients. (P<0.01) HRV in grade-3EH group showed significantly statistical differences in contrasting with grade -1 EH group and grade-2 EH group.2. HRV in EH patients who have left ventricular hypertrophy showed significantly statistical difference in contrasting with EH patients who have no left ventricular hypertrophy. The EH patients with Left ventricular hypertrophy also happen high ventricular arrhythmia.3. HRV in EH patients with ventricular arrhythmia showed significantly statistical difference in contrasting with EH patients without ventricular arrhythmia (P<0.01).ConclusionK The indices of HRV significantly reflect the impairment of sympathetic and parasympathetic function in the patients with EH. The reduced degree of HRV has a relationship with boold leves.There is a positive correlation between HRV and the severity ofEH.2> The indices of HRV reduce in the EH patients with LVH. Those who also have more chance to get with VA.It showed significantly reduced in EH, and it is the same in EH patients between with VAand without VA. There is a negative correlation between HRV and LVH,HRVandVA.3 > Analysis of HRV is a valuable and non-invasive method, it could be utilized quantitatively to predict the condiction and prognosis of EH patients.
