Objective:To study the long-term result of patients with laryngeal cancer treated by surgery and the prognostic factors.Methods:113 cases of laryngeal cancer treated surgically in the department of Otorhinolaryngology in the First Clinical Hospital of Shanxi Medical University from Jan 1998 to Mar 2002 have been retrospectively reviewed.Following the collected literature and also by referring to the record of the cases in our hospital,10 clinical factors(Age,Gender,Primary site, T status,N status,Clinical stage,Histology grade,Type of surgery,Type of neck dissection, Radiotherapy)were selected for the univariate and multivariate analysis in a Cox proportional hazard model.The Kaplan-Meier method was used to study the survival rate.Log Rank Test was used to compare of survival curves.Results:The overall cumulative survival rate was 78.5%at 3 years,69.3%at 5 years.In univariate analysis,T status(RR=1.589,P=0.008),N status(RR=1.818,P=0.003),clinical stage (RR=1.674,P=0.004)and histology grade(RR=2.236,P=0.001)were the factors that affected the prognosis of patients with laryngeal cancer,and they all were risk factors.In multivariate analysis,N status(RR=1.695,P=0.009)and histology grade(RR=2.231,P=0.003)were the main factors affecting the prognosis of patients with laryngeal carcinoma treated by surgery,and they all were risk factors.The survival curve indicated that as T status,N status and clinical stage increased,the survival rate of patients with laryngeal cancer at 5 years decreased.Conclusion:The overall cumulative survival rate for patients with laryngeal cancer treated by surgery is 78.5%at 3 years,69.3%at 5 years.As T status,N status and clinical stage increase,the survival rate of laryngeal cancer patients at 5 years decreases.The factors affecting patients with laryngeal cancer treated by surgery are T status,N status,clinical stage and histology grade,among which N status and clinical stage are quite obvious.
