Objective Recent clinical studies have shown that Hyperhomocysteinemia plays an important role in the progression of microvascular disease in patients with type 2 diabetes mellitus(DM).1.To survey the factors influencing the plasma Hcy in type 2diabetes.2.study the relationship between the Hcy and type 2 diabetic forepart nephropathy.3.Research the relationship of plasma homocysteiene(Hcy)and blood glucose,blood fat,kidney function,HbA1c,folic acids,vitamin B12etc.Method Group of type 2 diabetes:DM patients were diagnosed according to the DM diagnosis standards issued by WHO in 1999.including 85 patients from in-patient or outpatient clinic,who were suffering from type 2 diabetes and without vascular complication of the heart,brain,or peripheral vessels.We divided 85 patients with typeⅡDM into two groups:there are 43 patients in NDC group and 42 patients in DN group.In control,me have 75 people.We detected plasma levels of Hcy by chemiluminescence.We measured urine microalbum by immunity dispersion nephelometery,We use immunochemist to award the levels of serum Folic Acids and vitamin B 12.At last,we use spss 10.0 for windows to finish our stastics and analyses.Results:1.There are no signficant difference between two groups in age,sex,body mass index, waist-to-hip ratio,blood pressure and renal function.2.Plasma levels of Hcy and hyperhomocysteinemia(HHcy)in DN group were significantly higher than NDC groups,and plasma levels of Hcy and HHcy in DM groups showed obviously increasing compared with control group.(DN>NDC>CON).3.In the grounp with normal levels of Hcy,the incidence of DM with microvascular disease is percent of 23.33,But in the grounp with HHcy the incidence is percent of 63.33,and it is more than three times of normal group.4.relationship analysis between plasma homocysteine levels and plasma Folic Acid,vitamin B12,Our study support that there are negative associations between plasma levels of Hcy and plasma Folic Acid,vitamin B12.5.Speanman relationship analysis between plasma homocysteine levels and clinical parameters showed that plasma homocysteine levels was related with fast-ing plasma insulin levels and fasting plasma C-peptide levels.6.Risk factors of forepart nephropathy of diabete taking forepart nephropathy complication of diabete as dependent variable andcourse of disease,blood pressure,blood lipid,HbAlc,Hcy as independent variable,we did the mutiple stepwise regression The result showed that Hcy P=0. 039.It showed that plasma Hcy level in an independent factor of forepart nephropathy complication of diabete.Conclusion:1.Hyperhomocysteinemia of fast is positively correlated with forepart nephropathy complication of type 2 diabete.HHcy is regarded as a risk factor of DM patients amalgammating forepart nephropathy,suggesting that Hcy could contribute to the evaluation of the degree of forepart nethropathy complication in DM patients2.The levels of Hcy were affected by Folic Acid.Vitamin B12and disorder of metabolism.3.Fasting plasma insulin levels are significant related with fasting plasma homocysteine levels.
