AIM: To observe the histopathologic changes of the exocyst and"exo-adventitia"of hepatic hydatid cyst and to observe the expression of CTGF and TGF-β1 to evaluate the role of the connective tissue growth factor(CTGF)and transforming growth factor-β1(TGF-β1)in the formation of the pericystic layers of hepatic hydatid cyst.METHODS: We collected 45 cases of hepatic hydatid cysts and 1 case of splenic hydatid cyst. All of the samples were taken from the department of general surgery of the first affiliated hospital, medical college of Shi he zi University from Nov.2004 to May.2006 and all of them were diagnosed pathological professors referring to WHO standard. HE stain was used to observe the histopathologic changes and immunohistochemistry was used to observed the expression of CTGF and TGF-β1 in 45 pericystic layers of hepatic hydatid cysts.RESULTS: A special delaminated expression was observed in the pericystic layer of hepatic hydatid cysts. The"adventitia"could be sourced from fibrosing of the Glission system and hepatic vein system and the"adventitia"did not exist in the pericystic layer of splenic hydatid cysts. CTGF and TGF-β1 expressions in the pericystic layer near worm 55.56%,42.22% were lower than those in the pericystic layer near hepatic parenchyma 80.00%,88.89% (P<0.05).The expressions of CTGF and TGF-β1 in the pericyst and in the"adventitia"were coordinated, the difference between them were not significant(P>0.05).CONCLUSION: The expressions of CTGF and TGF-β1 were coordinated in the pericyst and in the"adventitia".TGF-β1 could regulate the expression of CTGF.Both of them were related to the formative process of the pericystic layer near hepatic parenchyma, which confirmed that the formative mechanism of the"adventitia"was tissue fibrosing and the formative mechanism of the pericyst and"adventitia"of hepatic hydatid cyst are different.
