| Obstructions of bile duct disease are common diseases of biliary tract diseases. The more complete and longer the Obstruction is, the higher the complication and mortality rate will be. The clinical performance differs by the patients' age, physical condition, etiopathogenisis, scope and locus of the pathological changes, thus, its manifestations are diversified. The patient may not seem to have obvious clinical symptoms at the early stage of incomplete obstruction, so diagnosis was based mainly on imageology such as ultrasound, CT, MRI, ERCP examination.At present, intervention technique is a new rising technology for biliary tract diseases, compared With traditional surgical method, It has the advantage of minimal insult, lower risk, and fewer complications; But still, some issues are to be resolved, such as the restoration of liver function the patency of the biliary drainage, and inflammatory response. The topic was a further study on these issues through intervention technique combined with channel-massage treatment.Objective: This thesis is set to study the liver function and clinical symptom improvements as well as bile secretion and biochemical changes before and after treatment, so as to approach the following advantages of Intervention technique combined with the channel-massage treatment: 1. Remove obstruction timely and effectively 2. Accelerate recovery of patients' liver function 3. Increase bile secretion and excretion 4. Improve clinical symptoms effectively 5. Ensure the smooth flow of biliary drainage and its maintenance after operation.Methods: Thirty-nine patients were divided into therapy group (n = 20) and control group (n = 19) by method of Random number table. The therapy group was treated by Intervention technique combined with the channel-massage treatment and the control group by interventional treatment. Observe the changes of liver function (ALT, TB, GGT) and clinical symptoms before and after treatment, the condition of bile biochemistry, biliary drainage and postoperative complications after treatment, and establish a database for statistical analysis.Results: 1,After treatment, TB, ALT and GGT of both group which compared with that before treatment increased significantly, and the results is of great statistical significance(P<0.05). 2,The therapy group patients enjoyed better liver function and clinical symptoms improvement than those from the control group; 24 hour bile drainage rate (376±96.2ml) was much more than that of the control group (281±85.4ml); 1.5 hour bile drainage rate after treatment (22±6.5 ml) was more than that of the control group (17±5.2 ml); Ca2+ of bile (1.81±0.32mmol/l) was lower than that of the control group (2.06±0.35mmol/l). Compared with the therapy group, these indexes above were all of statistically significance (P<0.05).Conclusions: 1. Intervention technique can release obstruction timely and efficiently and also has the advantage of minimal insult, high achievement ratio and as well as precise therapeutic effect; therefore, it can be used as a main treatment of some biliary tract diseases. 2. The channel-massage treatment can accelerate the restoration of liver function, reduce obstruction damage on organs. 3. The channel-massage treatment can significantly promote the bile secretion and excretion, and resolve the problem of the organization inflammation and dropsy after Intervention technique treatment. It can also solve the problem of re-obstructing or biliary drainage block after treatment; 4. The treatment can greatly release bellyache and abdominal distension after Intervention technique treatment. 5. Decrease the Ca2+concentration in bile, it would be of grate value to the third-level prevention of concretion, it also provide preliminary basis for our department studying with lithiasis third-level prevention. 6. Intervention technique combined with the channel-massage treatment can improve the clinical therapeutic effect of Obstructions of bile duct disease, it worth generalizing and clinical application. |
