Diagnostic Analysis Of240Cases Of Patients With Common Bile Duct Stones

Objective To research the role, serum biochemistry markers(ALT、GGT、ALP), imaging inspection and ERCP in the diagnosis of common bile duct stones.Methods Retrospective analysis of240cases diagnosed by ERCP in patients with common bile duct stones serum biochemical parameters, B ultrasound, the accuracy of MRCP and EUS diagnosis of common bile duct stones. The data collected were analyzed by SPSS20.0statistical package.Results Common bile duct stones occur in the older than60years of age or older group. Serum total bilirubin diagnosis rate was66.7%; ALT, GGT and ALP diagnosis rate is56.7%,72.9%,67.1%, increasing in GGT and ALP indicators consistent, at the same time as the diagnosis of common bile duct stones more sensitive indicators; GGT increased as the diagnosis of the main indicators of significance. Three imaging methods in the diagnosis of type-B ultrasound rate of81.6%, MRCP diagnosis rate of89.4%, the EUS diagnosis rate was97.4%, type-B ultrasound compared with the MRCP group, P<0.05, the difference between them was statistically significance; type-B ultrasound compared with the EUS group, P<0.01, the difference was statistically significant; the MRCP group compared with EUS group, P>0.05, the difference was not statistically significant; type-B ultrasound under normal circumstances, due to a fast, non-invasive and other advantages, is still the preferred method of diagnosis, but the diagnostic accuracy of the lowest in all imaging diagnostic tool; MRCP diagnosis with high accuracy in the B-very intuitive for clinical image data; EUS diagnosis in line is the highest rate of all above.Conclusion Serum biochemical markers, imaging methods (Type-B ultrasound, MRCP, EUS) and ERCP diagnosis of common bile duct stones, each with a different role, combined with the application can effectively improve the diagnosis rate. ERCP is still the diagnosis of common bile duct stones gold standard, an irreplaceable role.
