Objective:Treatment according to syndrome differentiation is the basic feature of TraditionalChinese Medicine (TCM). So in the research of knee osteoarthritis (OA), it is important toexplore the rule and standard of pattern of syndrome of TCM. Our study is a prospectivestudy. We study the OA syndrome and pattern ofsyndrome of TCM by statistics method inorder to find out the contact of OA syndromes and patterns of syndrome of TCM. Then wecan find out the rule and standard to classify the pattern of syndrome of TCM of OA. Andwe attempt to find out if there is correlation between patterns of syndrome of TCM andx-ray symptoms.Method:We investigated 120 cases from out-patient clinic of Traumatic and Orthopaedicshospital affiliated to Guangzhou University of TCM for a prospective questionnaire survey. Then we take x-ray pictures of the knees of OA and analyze. All data were input intoSPSS12.0 database and analyzed by clustering analysis and factor analysis.Results:105 cases conform to the study can be classified into 2 clusters by clusteringanalysis: the course of OA is longer in cluster 1, and the syndromes are deficientsyndrome for the most part; the course of cluster 2 OA is shorter compare to cluster 1, andthe syndromes are mostly sthenia syndrome. There were 8 factors resulted by factor analysis. And there are 2 factors can bedefined: factor 1 is deficient factor, factor 2 is cold factor. And both of these 2 factors areopposite to the stagnated blood symptom. It doesn't have manifest or specific association between the symptoms in KOA and the change of x-ray. So as to the correlation between pattern of syndrome of TCM and thechange of x-ray.Pathological changes of the patella-femur joint and medial part of knee joint in OAwere frequently encountered in the KOA x-ray pictures. The angle of tibia and tibia-femurangle were correlated with the stenosis of the medial joint space. The high density imageof upper-brusa of patella appears frequently in the cases.Conclusion:1. Classified by the clustering analysis, the KOA patients can be divide into 2 clusters:deficient syndrome and sthenia syndrome. The course of disease in deficient syndrome islonger than sthenia syndrome.2. 2 factors can be defined after factor analysis: Factor 1 deficient factor, factor 2 coldfactor and the sluggish symptom which was opposite to the 2 factors. These threeelement are similar to the three type of syndrome which were described in the booknamed of the standard of diagnosis and curative effect of Chinese medicine disease andsyndrome.3. Clustering analysis and factor analysis are very useful to classify the symptom ofdisease for pattern of syndrome of TCM research and clinical diagnosis.4. it doesn't have manifest or specific association between the type of syndrome ofChinese medicine in KOA and the change of x-ray.5. The pathological changes of patella-femur joint, medial part of knee joint andsurrounding soft tissue of knee are very important in KOA pathology.
