Objective: (1) To investigate the clinical and immunological characteristics of first-degree relatives of classic T1DM and LADA. (2) To assess the pancreatic beta-cell function in non-diabetic first-degree relatives of classic T1DM and LADA.Objects and methods: Simplified OGTT were performed in 340 cases of first-degree relatives of patients with classic T1DM and 209 cases of first-degree relatives of patients with LADA. The differences of clinical characteristics were observed. GADA and IA-2A were measured in the two groups. IVGTT were performed in 39 normal controls and 46 first-degree relatives. Among them, 9 were GADA positive and 9 were GADA negativefirst -degree relatives of patients with LADA, while 12 were GADA positive and 16 were GADA negative first-degree relatives of patients with classic T1DM.Results: (1) The waist-hip ratio of first-degree relatives of ptients with LADA is higher than that of classic T1DM (0.87vs0.84, P<0.05). Regression analysis showed that age,BMI,WHR were significantly correlated with FPG,2hPG of first-degree relatives of patients with LADA (P<0.01). GADA detection rate was similar in two groups, and it was higher than the IA-2A detection rate of first-degree relatives of patients with classic T1DM (P<0.05). (2) Compared with the control group, The FPIR and first phase insulin peak secretion averages were lower in the antibody positive group of first-degree relatives of patients with LADA and classic T1DM (P<0.05). Compared with the control group, HOMA-IR was higher in the antibody negative group of first-degree relatives of patients with LADA (P<0o05).Conclusion: (1) Aging and augmentation of BMI,WHR were increased risk factors of FPG,2hPG of first-degree relatives of patients with LADA. (2) In the first-degree relatives of patients with LADA group, insulin resistance with insulin secretion defects were detected. In the first-degree relatives of patients with classic T1DM group, marked islet cell secretory dysfunction was observed.
