Objective: Gastric carcinoma was one of malignant digestive tumors in our country. Patients with gastric carcinoma died from metastasis of gastric carcinoma. It has been known that combined biological effects of multiple genes are involved in the carcinogenesis of gastric tissues and metastasis of gastric carcinoma. The purpose of this study was to investigate the expression of D2-40, VEGF-C, VEGF-D and CD44V6, nm23 in gastric carcinoma to explore their role in lymphangiogenesis and metastasis of gastric carcinoma and try to provide referential indexes to the treatment of gastric carcinoma. To study the expression of nm23 and CD44V6 and their roles in the mechanism of the oncogenesis and metastasis of gastric carcinoma.Methods: 1 The expression of D2-40, VEGF-C, VEGF-D were examined by s-p immunohistochemical (IHC) and HE stain in 63 carcinoma and 35 adjacent carcinoma and 23 normal control tissue.2 The expression of nm23 and CD44V6 in 40 carcinoma and 20 adjacent carcinoma and 15 normal control tissue were quantitatively measured by using flow cytometrye technique (FCM).3 SPSS 11.5 for windows was applied to analyze the results of experiment.Results: 1 D2-40 expression by counting Lymphatic vessel density(LVD) in different gastic tissues: LVD was remarkably higher in gastic carcinoma group and adjacent carcinoma group than that normal control group (P<0.05), But LVD have no remarkably difference between gastric cacinoma group and adjacent carcinoma group (P>0.05). LVD were uncorrelated with age, gender, differentiation degree(P>0.05); but were correlated with TNM clinic stage, lymphtic metastasis and infiltrating depth(P<0.05).2 The expression of VEGF-C,VEGF-D protein in different gastric tissue: VEGF-C positive staining was observed in 54 cases of 63 carcinoma tissues; and 9 cases were observed negative staining. VEGF-D positive staining was observed in 55 cases of 63 carcinoma tissues while 8 cases were observed negative staining. Statistical analysis revealed that the expression of both of VEGF-C and VEGF-D were higher in tumor group than that in adjacent carcinoma group and normal control group(P<0.01). Both of VEGF-C and VEGF-D expression were not correlated with age, gender, differentiation degree(P>0.05); but were correlated with lymphtic metastasis, TNM clinic stage and infiltrating depth(P<0.05).3 Relationship of the expression of LVD, VEGF-C, VEGD-D protein in gastric carcinoma group: LVD was higher in VEGF-C positive staining group than that in its negative staining group. LVD was higher in VEGF-D positive staining group than that in its negative staining group. VEGF-C was positively correlated with VEGF-D (P<0.01).4 The protein contents of CD44V6 and nm23 by FCM: CD44V6 expression in gastric carcinoma group was higher than that in adjacent carcinoma group and normal control group (P<0.01), CD44V6 expression in adjacent carcinoma group was higher than that in normal control group(P<0.01). The nm23 expression in gastric carcinoma group was lower than that in adjacent carcinoma group and normal control group (P<0.01), but was no difference in adjacent carcinoma group and normal control group (P>0.05).Conclusion: 1.LVD was correlated with gastric carcinoma lymphatic metastasis, D2-40 was a better index to detect gastric carcinoma lymphatic metastasis. D2-40 has clinical value to gastric carcinoma diagnosis, relapse and metastasis.2.VEGF-C and VEGF-D may prompte cancer cell invasion and metastasis by facilitating lymphangioenesis in gastric carcinoma.3.Combined detection of VEGF-C,VEGF-D and D2-40 expression have showed definitely clinical value on relapse and metastasis, estimation prognosis, formulating individualized comprehensive therapy plan in the gastric carcninoma. They might indicate lymphatic nodal metastasis of gastric carcinoma. 4.nm23 protein suppresses the genesis, infiltration and metastasis of gastric carcinoma, while CD44V6 plays an active role in the genesis and metastasis of gastric carcinoma. Thus a low level of nm23 and a high level of CD44V6 may indicate a poor prognosis of gastric carcinoma.
