Combined Therapy Of Radiofrequency Ablation With Hepatic Artery Embolization In Animal Model Of Liver Tumor: An Efficacy And Safety Study

Objective:To investigate the therapeutic efficacy and safety about the combined therpy of RFA with TAE in rabbits with VX2 liver tumor.Material and methods:1.Under the guidance of CT, VX2 tumor tissues were implanted into the rabbit's liver by the method of percutaneous transhepatic punction .Two weeks later the animal model was proved definitely by CT when the tumor had grown into the size of 2cm in diameter. 2.Thirty-six rabbits with tumor in liver were assigned to 4 groups randomly (n=9). group A: only treated with RFA; group B,C,D: RFA were carried out at 1d,4d,7d respectively after TAE by Iodized Oil. 3.The areas of coagulation necrosis were observed at 7d after treatment in each group. The macroscopic samples and pathology were also examined at 1d,4d,7d after treatment. 4.The ALT and UREA levels of the experimental animals were explored before the therapy of RFA or TAE+RFA and at 1d,4d,7d respectively after the therapy.Results:1.After RFA or TAE+RFA ,The rabbits ,especially in group B and C,decreased their appetite apparently.The majority of them were in depression and drowsiness and moved little. Then they restored to the pre-therapeutic condition in 3d in group A,5~7d in other groups. 2.No rabbits died in group A and D.5 rabbits were dead in group B and C(3,2 rabbit respectively). And all the death of rabbits happened in 24h after therapies. 3.The mean ablation areas were2.99±0.18cm~2,3.67±0.32cm~2,3.65±0.29cm~2,3.53±0.16 cm~2 in group A,B,C,D and respectively.the ablation area in group B,C,D was much larger than in group A(p<0.05).There were no statistical differences between group B,C,D.(p>0.05). 4.ALT levels were the highest at the first day in all groups.Their mean levels were 178.4±49.2IU/L,385.0±213.1IU/L,289.2±192.6 IU/L,208.2±65.5 IU/L in group A,B,C,D respectively ,However they had decreased obviously after 4d. The difference was significant at the first day between group B and A or B and D respectively(p<0.05), but not significant between group B and C (p>0.05);There were differences at the first day between group C and A or C and D also(p<0.05), not significant between group D and A (p>0.05). There were no differences between each group in the 4th and 7th. it was not statistical significance of UREA levels between each group.(p>0.05).Conclusion: 1.The combined therapy of RFA with TAE, which is less invasive and can increase necrosis ablation areas obviously,is an effective therapertic method for hepatic neoplasms. 2.It was impossible to make tumor necrotic completely in ablation area with RFA alone,especially the tumor tissues adjacent to the large vessels.Complete necrosis happened with TAE+RFA in ablation area,including the peripheral part of ablation area in a few animals. 3.RFA at 1d,4d after TAE might impact liver function severely. it was coincident with increase of ALT;RFA ,at 7d after TAE is relatively much safer. 4.there was no obvious impact to renal function of group A,B,C and D.
