Objective: To observe whether hypertonic solution chloride hydroxyethyl starch injection(HSH)exerts protective effect on the lung tissue damage of rabbits after resuscitation from pulmonary contusion and hemorrhagic shock .Methods: 24 adult healthy New Zealand rabbits of either sex were randomly divided into four equal groups,6 rabbits in each group :the Control group,the RL group,the HES group,the HSH group. Pulmonary contusion models with hemorrhagic shock were reconstructed,and then resuscitated these rabbits with different liquids. To draw the arterial blood at 3 time ( before bloodinglet,1 hour after resuscitation,4 hour after resuscitation) to measure blood gas analysis,wet/dry (W/D) ration of lung,and neutrophils percentage in bronchoalveolar lavge fluid (BALF) were detected. Blood serum levels of superoxide dismutase (SOD), malondialdehyde (MDA), and the myeloperoxidase (MPO) in lung tissure were detected using chemistry colorigical. To observe changes of the lung tissure under light microscope.Result: hemorrhagic shock can be remedied by 3 methods. Resuscitation with HSH can retrieve hemodynamics quickly and maintain stable for a lomg time, and the protective effect in the function of lung is obvious. Resuscitation with HES significantly reduced bronchoalverlar lavage fluid neutrophil counts(P<0.01),lung edema,and the dgree of histopathologic injure , lowered MDA and MPO activity (P<0.05,P<0.01) and increased SOD (P<0.05,P<0.01) compared with resuscitation with Ringer's lactate at various experimental time. The effect of resuscitation with HSH is more obvious.Conclusion: 1 The effect of resuscitation with HSH in the retrieving and maintaining haemodynamics stable is quick and durative. 2 HSH can effectively attenuate lung neutrophil sequestration in the lung injury, reduce oxygen free radicals levels. 3 Hypertonic saline hydroxyethyl starch can be used in the treatment of hemorrhagic shock with pulmonary contusion.
