| Background:In this time, with the great improvement of our substance living standard, Obesity become a worse problem for overnutrition, being short of athletic sports. There are several hundreds millions of peason face this problem. Obesity can not only cause hypertension, hyperlipemia, cardiac disease, diabetes, but also influence the beauty of body. We used to taking operations in plasty surgery, such as baggy eyelids plasty, liposuction technique and so on. But a set of problems following these ways such as bleeding, infection, scarring, and fat embolism make the patients aren't satisfied with. People are persuiting a way to cut the fat that without operation and little trauma.During the last few years, for it's effectiveness of fat degradation, phosphatidycholine was used for fat-dissolving by local injections in US and Europe. Phosphatidycholine, used named lecithin, is a primary composition to construct the cellular membrane. It has been greatly used for liver disease and disease of blood vessel for a long time, such as hepatic adipose infiltration, hyperlipemia, fat embolism, atherosclerosis and so on, which had made success in animal test and clinical application. There isn't enough experiment study on injectable phosphatidycholine used for fat-dissolving, so that the effectiveness, sefaty and mechanism is not very clear yet. Our rearch work is to make some primary study for injectable phosphatidycholine used for fat-dissolving by control animal experiment.Obieetive To evaluate the efficacy, mechanism and safety of the injectable phosphatidycholine Lipostabil used for fat-dissolving via control animal experimental studies.Methods Twelve Guangxi Bama miniature pigs were divided into study group and control group in random, 6 cases in each group, and the number of male and that of female was equal. The locations of one side of the pigs of study group were given endermic injection, injection into subcutaneous fat and intramuscular injection with 0.5ml injectable phosphatidycholine Lipostabil. Being served as control, 0.9% saline solution of equal volume was given to the pigs of control group in the same way, corresponding location of another side were designed for biank control. Gross observation, histology and studying the side effects to the whole body was applied to analyze the results. The fat layer thickness of the location that given injection and the biank control location were measured by ultrasound after all the injections.Results Obvious destroying to fat tissue was observed. One week after the first injecton to fat tissue with phosphatidylcholine, we got the location fat tissue for histology and could find infiltration of inflammatory cells in injection location of fat tissue in study group pigs. In the location, the adipocytes became distorted, corrugativus and losted the normal form. A great many inflammatory cells could be found in the injection location of fat tissue after 2-3 injection. The structure of fat tissue was badly destroyed, and the adipocytes were phagocytized in great numbers, even disapeard in some area. When the forth or fifth injection was finished, in the location, the inflammatory cells reduced, and the fibrous degeneration could be found. HE staining was red. In the injection location of fat tissue of the control group pigs, the obvious struction destroyed was not found. In the study group pigs, the fat layer thickness of the injection location was thiner than that of the blank control location(p<0.001), and in control group pigs, the thickness difference was not distinct(p>0.05).The damage to the skin was also found in skin location injected with phosphatidylcholine. After each time injection, slight red swelling of the skin was found in the injection locaton of skin in the control group pigs, and was extinct in about 1 hour. In the study group pigs, obvious red swelling was found in the injection location of skin soon after the first injection, and disappeared slowly in about 10-24 hours. Some pigs' skin of injection location became to necrosis after the second injection, and the epidermis became black. The skin necrosis was found in all study group pigs after the third injection into skin. The destroy to the muscle tissue was also found in the injection location, one week after all 5 injection to muscle with phosphatidylcholine finished. There wasn't experiment animal death happened after the local injection with phosphatidylcholine. The animals' life habit, food and drink,sleep didn't changed after the injection experiment. Dyskinesia didn't happen to the animals.Conclusion In this experiment, we could found that the injectable phosphatidycholine Lipostabil was effective for fat-dissolving to some extent, which made inflammation and fibrous degeneration happened in the fat tissue. The obvious side effects to the whole body was not found.The phosphatidycholine do not have tissue specificity. It could make the mummification necrosis to the epidermis, breaking down of the dermal collagen by injected into the skin hypodermis directly and destroyed the muscle fiber by injected into the muscle directly.Even the phosphatidylcholine Lipostabil was effective in local injection for fat-dissolving, but we need more research in the actual effectiveness and safety before it being used clinically for plasty. We should take much care of in local injection of phosphatidycholine(Lipostabil)for fat-dissolving, and master the injection location and layer exactly. |
