Purpose: To observe therapeutic effects of electroacupuncture(EA) with different frequencies and western medicine on acute gout arthritis .To observe the time of inducing analgesic effect and the effect-lasting time ,and evaluate the analgesic effect. The level of blood uric acid and IL-6 was measured before and after treatment, settling the foundation for further study.Methods: 60 cases of acute gout arthritis were divided randomly into medication group (group C),EA with 2Hz (group B) or 100Hz groups (group A) ,20 cases in each group .All the patients in group A and group B were treated by EA in the same way but different parameters of stimulation were applied. The operative method Was as follow: needles were inserted into acupoints Zusanli(ST36) and Sanyinjiao(SP6),then electric stimulation was given with Han' EA apparatus with the parameters of frequency of 100Hz and 2Hz, wave width of 0.6 ms and 0.2 ms for group A and group B respectively, strength of current of 0.5mA at the beginning, which was increased to 1mA and to 2mA after another 10 min. So the stimulation was applied for 30min in total. Patients in group Cwere treated with 25mg indometacin and 100mg of allopurinol three times a day through oral intake with 6days as one therapeutic course. After one therapeutic course, to observe the time of inducing analgesic effect and the effect-lasting time ,and evaluate the analgesic effect. The level of blood uric acid and IL-6 was measured before and after treatment. Results (1) Clinical Efficacy: The total effective rate in Group B was better than that in Group A and Group C, showing significant difference (P<0.01), while the difference between Group A and C was insignificant (P>0.05), suggesting that EA treatment with frequency of 2Hz could obtain the best efficacy. (2)Effects on Pain Scores: comparison between the three group in pain scores after treatment showed significant difference (P<0.01), with the score in Group B superior to that in the other two groups, showing significant difference (P<0.01),and that in Group A better that in Group C, also showing sinificant difference (P<0.01). All these suggested that EA of frequency of 2Hz had the best efficacy in stopping pain. (3) Comparison of Initiating and Sustaining Time of Analgesic: The initiating and sustaining time of analgesic in groups treated by EA were shorter and longer than those in the Western medicine treated group, respectively , showing difference of statistical significance (P<0.01); when the effect in Group B was compared with that in Group A, suggesting that the analgesic effect of EA treatment was superior to Western medicine, and low frequency EA was superior to high frequency EA in analgesic initiation and duration . (4)Serum Level of Uric Acid and IL-6:Serum level of uric acid and IL-6 were reduced significantly after treatment in all the three groups(P<0.01). Comparison between them showed the result in Group B and C was significantly better than that in Group A(P<0.01),but the difference between Group B and C was insignificant(P>0.05).Conclusion EA is an effective method for treating acute gouty arthritis, and its efficacy is directly related with the frequency applied. The method, Obvious curative effect. The patients suffer litter and accept this method easily. And this method is very simply and worth speeding.
