Objective: To investigate the new way of treating tubulointerstitial fibrosis(RIF)by the Regulating Collateral and Eliminating Turbidity Liquid(REL). Consequently to research the mechanism of tubulointerstitial fibrosis which is related to endothelin level, and the effect of Regulating Collateral and Eliminating Turbidity Liquid of protecting tubulointerstitial fibrosis. Methods: The experiment is based on the modle of unilateral ureteral obstruction leading tubulointerstitial fibrosis, divid 48 Waster rats into 3 groups: Sham-oparation ,UUO and UUO+REL. Four rats from each groups were sacrificed at 3,10,20,30days after UUO. Befor the scarification limosis serum was collected ,and the endothelin levelserum, serum creatinine, urea nitrogen and coagulation mechanism were measured. And in the clinical observation, choose 60 patients who has been chronic kidney function exhaustion but still not under dialysis yet, give them REL as the main prescription with variation in quantity. And in one month, observe the conversion of endothelin level in serum befor and after the treatment, and the improvement of coagulation mechanism and kidney function. Results: The serum creatinine, urea nitrogen and endtholin level of UUO group increased significantly compared with those of Sham group(p<0.01).Those of UUO+REL group increased too, but them are lower than those of UUO, the difference is obviously. By comparing the patients before and after the treatment, the endtholin level decreased obviously, and their kidney function all improved more or less, the efficiency rate is 10%, and the totoal efficiency reached 85%(p<0.05). Conclusions: The kidney collateral stasis is the primary pathological basis of RIF, blood-nourishing, blood-promoting, collateral-regulating, turbidity-eliminating therapy is effective on it. And the Regulating Collateral and Eliminating Turbidity Liquid could influence the metabolizability of endothelin, decrease the level of endothelin, improve the kidney function and delay the developemtn of tubulointerstitial fibrosis.
