Objective;To detect the risk factors of stomal recurrence cancer in patients with total laryngectomy.Methods;From 1990 to 2006,270 patients with laryngeal carcinoma were treated by total laryngectomy.Among cases(202 males and 68 females,ranging in age from 35 to 78 years old,Stage T2 61,T3 156,T4 53), 35 cases were stomal recurrence cancer(12.9%)within two--year total laryngectomy.The risk factors of stomal recurrence cancer were analyzed with COX model.Results;The rate of stomal recurrence cancer was 21.7%(5/ 23)in subglottic cancer,29.0%(9/31)in transglottic cancer,13.4%(21/ 156)in stage T3,and 26.4%(14/53)in stage T4.Above 4 factors were showed correlation with the stomal recurrence cancer.Conclusion;the stomal recurrence cancer after total laryngectomy is the most devastating late complicantion.Its development is closely associated with subglottic extension, extralaryngeal spread of the primary tumor(stage T3,T4).
