Analysis For Stomal Recurrence Cancer After Total Laryngectomy

Objective;To detect the risk factors of stomal recurrence cancer in patients with total laryngectomy.Methods;From 1990 to 2006,270 patients with laryngeal carcinoma were treated by total laryngectomy.Among cases(202 males and 68 females,ranging in age from 35 to 78 years old,Stage T₂ 61,T₃ 156,T₄ 53), 35 cases were stomal recurrence cancer(12.9%)within two--year total laryngectomy.The risk factors of stomal recurrence cancer were analyzed with COX model.Results;The rate of stomal recurrence cancer was 21.7%(5/ 23)in subglottic cancer,29.0%(9/31)in transglottic cancer,13.4%(21/ 156)in stage T₃,and 26.4%(14/53)in stage T₄.Above 4 factors were showed correlation with the stomal recurrence cancer.Conclusion;the stomal recurrence cancer after total laryngectomy is the most devastating late complicantion.Its development is closely associated with subglottic extension, extralaryngeal spread of the primary tumor(stage T₃,T₄).
