Background Colorectal adenoma-adencarcinoma sequence is the recognized hypothesis of tumorigenesis of the large bowel cancer.Some Researches show that the TGFβ1 pathway involved in the tumorigenesis of the large bowel cancer.But the study of the expression of the TGFβ1 signal pathway protein in the large bowel adenoma, especial the high risk large bowel adenoma,is rarely.Objective To retrospective analysis the clinicopathlogical characteristics of the high risk large bowel adenoma,and compare the expression of the protein of the TGFβ1 signal pathway in the normal colorectal tissue,colorectal adenoma and colorectal cancer;To investigate the relationship between the clinicopathologic features of the high risk large bowel adenoma and the expression of the all protein of the TGFβ1 signal pathway.Methods Collect the cases of the patients with large bowel polyps who underwent polypectomy between 2000 and 2006 at People's Hospital of Luan in Anhui Province, and register the data of the clinicopathologic features.Then classify the polyps according the diagnostic criteria in "the Guidelines for Colonoscopy Surveillance after Polypectomy" in 2006.Choose 110 large bowel polyps,including 50 case of high risk large bowel adenoma,30 case of low risk large bowel adenoma,and 30 case of non-tumor type polyps,at the same time,40 case of large bowel cancer,20 case of health samples as controls.The paraffin-embedded tissue specimens of the three groups are made by serial sections.Identify the expression of TGFβ1,TGFβRⅠ,TGFβRⅡ, Smad3,Smad4 and Smad7 through S—P immunohistochemistry of three groups,and investigate the relationship between the clinicopathologic features of three groups and the expression of the TGFβ1 signal pathway protein.Especailly,compare the expression of TGFβ1 signal pathway protein between the high risk large bowel adenoma and the large bowel cancer in the Duke's A stage.Results The expression of TGFβ1,TGFβRⅡ,Smad3,Smad4 and Smad7 of the three groups,the health control group,the large bowel cancer group and the large bowel polyps group,is significantly different(P<0.05),except TGFβRⅠ(P>0.05).The expression of the TGFβ1 signal pathway protein is that the expression of TGFβRⅠis associated with the number of the polyps,and the expression of TGFβRⅡis associated with the size or pathologic type of the polyps between the clinicopathologic features of the large bowel polyps group.And the expression of the TGF1 signal pathway protein is that the expression of TGFβ1 is associated with the patients' sex,and the expression of Smad3 is associated with the patients' age between the clinicopathologic features of the large bowel cancer group.The relationship between the clinicopathologic features of the high risk adenoma group and the expression of and the expression of the TGF1 signal pathway protein is that the expression of all the indexes is not associated with the clinicopathologic features.The expression of TGFβ1,Smad3,and Smad7 of the high risk adenoma group and the large bowel cancer in the Duke's A stage is significantly different(P<0.05),except TGFβRⅠ,TGFβRⅡ,and Smad4(P>0.05).Conclusion According to the diagnostic criteria of the high risk large bowel adenoma, analyses the risk stratification of the patients with large bowel polyps,especially with the large bowel adenoma,have very important clinic significances.The expression of the proteins in the TGFβ1 pathway,as TGFβ1,TGFβRⅡ,Smad3,Smad4 and Smad7 is significantly different in the health sample,large bowel polyp and large bowel cancer groups,which indicate TGFβ1 pathway may relate to the tumorigenesis of the large bowel cancer.The expression of TGFβ1,Smad3,and Smad7 is significantly different in the high risk large bowel adenoma,and large bowel cancer in Duke's A stage groups, which provide the clue for researching the role of the TGFβ1 pathway in the cancerization of the large bowel adenoma.
