Objective To Study the clinical significance of computed tomography(CT) and three - dimensional computed tomography reconstruction (3D-CT) in diagnosis and treatment of tibial plateau fractures. Methods 202 cases of tibial plateau fractures were examined preoperatively under X - ray , CT scans and 3D–CT. Based on CT images , we divide three regions(columns) in the tibial plateau ,and then classify the fracture, compute the incidence of posterior column fractures. Compared the results with results of X-ray classification and the discoveries in the operations, assessed the value in diagnosis and treatment of tibial plateau fracture. Results In total 202 cases, 21 cases had different results between X-ray and CT :5 tibial plateau fractures was not found on X-ray films but on CT scans, the classification for 16 fractures was wrong.. There were 47 posterior column fractures of tibial Plateau, and the disease incidence was 23.2%. The classification coincidence between X-ray and CT was 89.6%. Conclusions The image of 3D-CT reconstruction can make the tibial plateau fractures visualized from different view, which can provide great advantage to fracture classification. Posterior column fracture of tibial Plateau was not seldom, CT scans could diagnosis this fracture, three-column method could help make an more appropriate treatment plan.
