Objective:To investigate the clinical efficacy and anatomic base of internal fixation by posterior approach in treatment of tibial plateau fracture.Methods:15 cases of the tibial plateau fractue underwent open reduction and internal fixation wth T-plate or L-plate through a posterior approach. All the cases were followed up and the clinical effects were analyzed.Results:The follow-up duration was ranging from 14-16 months. All fractures got united with an average radiographic bone union and an average full weight-bearing time of 18 weeks. By the radiological Rasmussen criteria for fracture reduction, the mean score immediately after surgery was 16.8 points (from 14 to 18). the mean HSS(The Hospital for Special Surgery)score of all these patients at 12 months postoperationly were 84.9(ranging from 60 to 96). There were no significant differences both between TPA and PA after operation and 12 months postoperatively.Conclusion:Open reduction and internal fixation with buttress plates via a posterior or anterior plus posterior approaches is appropriate for posterior column tibial plateau fractures.
