Objective To evaluate the clinical effects of cystectomy with needle aspiration and complete removal of endocyst via a comparative analysis of patients' data of pulmonary hydatid cyst. Methods From June 1 965 to December 2 004, 210 patients were undergone cystectomy with needle aspiration and 141 patients were undergone complete removal of endocyst out of total 351 patients with non-ruptured pulmonary hydatid cyst, and a retrospective analysis was undergone on patients' data including clinical feature,duration of operation,postoperative hospital stay,duration of postoperative drainage, postoperative first day drainage volume, postoperative complication and recurrence, respectively. Results There are no significant differences on duration of operation,postoperative hospital stay, duration of postoperative drainage, postoperative first day drainage volume and postoperative complication in two groups respectively (P>0.05); However, there is significant difference in postoperative recurrence (P<0.05), 20 recurrences after cystectomy with needle aspiration and 4 recurrences after complete removal of endocyst during 2~20 years of follow-up. Conclusion Complete removal of endocyst is the ideal surgical procedure in the treatment of pulmonary hydatid cyst, though it requires a higher surgical technique and its duration of surgery is a little bit longer than cystectomy with needle aspiration, it gets a satisfactory curative effect with proper handling and reduces postoperative recurrence; Each operation has its advantages and disadvantages, therefore, specific operation method should be applied according to cyst feature and surgical technique.
