In otorhinolarygology field ,There are many cartilaginous tissue defecti-ons which effect seriously the pations'health, functional recovery and facial. Three methods are usually useed in cartilage repair . They are autot-ransplant ,allotransplant and artificial materials.The main problem for thesemethods is the damage to the donor, no suitable shape for the recipient, rejection from the recipient ,lack of enough cartilage and foreign body reac-tion ,infection and degradation for the artificial materials. In recent years ,tissue engineering technology provides a new way for solving these proble-ms. Tissue engineering is a multidisciplinary science merging with cell boil-ogy, material science, engineering and chemistry. Its basic idea is to enabl-e the organization of 3-dimmentional and functional tissues or organs. But tissue engineering technique are associated with problems of difficult moldin-g for multiplicity shape tissue or organs. And it is difficult to reconstruct the laryngotracheal framework according to individual anatomic structure. Therefore, it has become a main aim to find a new approach to resolve these problems.This study attempt to repair and reconstruction the defects of laryngontracheal framework with organ-print technique, constructing mult-iplicity shape cartilage prosoma with different biomaterial framework and tooting shaping methods, and repairing tracheal defect .It can provid theory and experimental basis for pate cartilaginous tissue repairing and reconstruc-tion.Materials and methods1. Experimental study on calcium alginate gel mechanics character and bio-compatibility1.5% alginate gels mixed with different concentration of calcium solution 100,200,300,400,500mmol/L , solidified for minutes to form high 25mm, diameter 13mm stylolitic calcium alginate gel. Materials test machine measured the max enduring load and drawed the curve diagram. Subculturi-ng the third cartilage cells mixed with alginate gels ,then it with different concentration of calcium solution 100,200,300,400,500mmol/L was added separatedly and solidified for 15 minutes to form stylolitic calcium alginate.then cultured in the DMEM complete culture solution.After 1weeks, 2 weeks, 4 weeks, 6 weeks culturing in vitro, gross and histological obser-vation were done. Human nasoseptal chondrocytes were mixed with conce-ntration of calcium solution 300mmol/L, then cultured in vitro for 8weeks,Gross and histological observation were done . 2. Experimental study on computer technology aided to construct thyroid cartilageA continuous lamellar CT presentation was selected in the middle of all the patients of good thyroid cartilage and made three-dimens ionalgeometric model with iatro-fault deal software which was made in shanghai jiaotong university. then it was exported with STL file format.The data was inputedinto rapid prototyping machine to get three-dimens model.The cavity blockwas filled with calcium alginate gel and cartilage cell ,then was sintered toproduce the bone scaffold. The cellscaffold was embedded in animal body. After 8 weeks, gross and histological observation were done.3. Experimental study on computer technology aided to construct cartilagin-es tracheales rings and repair the defect of trachea antethecaThe method of computer technology aided to construct cartilagines trache-ales rings model was the same of thyroid cavity block. The flowsheet of gel pouring mole modeling technique made tracheales rings withβ-TCP. A full-thickness defect (2cm×5cm) was created in tracheal rings 3 through 5 in adult dog. Experimental group: the defect of trachea was repaired with cervical muscles. The skin tube was sutured to two tracheal cartilage ring to keep it from collapsing.Control group: the defect of trachea was repaired only with cervical muscles.All animals were followed. The breath and swal -lowing functions of the dogs were observed.Experimental group dogs survi-ved to 8 weeks. After 8weeks, the repairings were analyzed by laryngofiber-scopy, gross observation,computer tomography scan and hematoxylin and eosin(HE) staining of tissue sections.Control group dogs survived to 8 weeks.After 8weeks, they were analyzed by laryngofiberscopy, gross observation. Results1. Experimental study on calcium alginate gel mechanics character and biocompatibilityThe concentration of calcium solution can enhance the intensity of calci-malginate gels .By 1weeks, 2weeks, 4weeks, 6weeks culturing in vitro, HE and Masson staining were made to understand the growth condition ofcartilage cells. By culturing, cartilage cells can abundantly amplify,and hold activity and proliferation in the concentration of 100 mmol/L,200 mmol/L and 300 mmol/L calcium alginate gels. At 6 weeks, it has becomes alike cartilage.Cartilage cells can not hold better activity and proliferation in the concentration of 400 mmol/L and 500 mmol/L calcium alginate gels after 1 weeks. So, the concentration of calcium solution may increase to 300 mmol/L, which has no influence on the growth of cartilage cell, and can enhance the intensity of calcium alginate gels.By 8weeks culturing, human nasoseptal chondrocytes can abundantly amplify and hold activity and prolif-eration in the concentration of 300mmol/L calcium alginate gels.2. Experimental study on computer technology aided to construct thyroid cartilageIt has made successfully the cavity block of resinous thyroid cartilage with Rapit Prototyping. Subculturing cartilage cells mixed with the concentr-ation of 300mmol/L calcium alginate gels to plastotype according to the moul-d in vitro.After 8 weeks culturing in ault rabbit body, the shape of embeddi-ng has disappeared.Pathological section showed that it had cartilage lacuna algin frame materials had disappeared.3. Experimental study on computer technology aided to construct cartilagines tracheales rings and repair the defect of trachea antetheca.The experimental animals survived well and breathed normally. CT scan showed that the implants were fixed perfectly. Laryngofiberscopy showed thatthe reconstructed airways had no narrow. Fibrolaryngoscope can pass through successfully. Gross observation showed that the frame material had mixed with trachea circum-tissue .There was little granulation tissue in the lumina. Pathological section showed that circum tissue had grew in the pores of Frame materials. One control group animals died for cutaneous emphysema on the evening after operation. After dissect showed that the reparing secti-on had callapsed, the reconstructed airways has became narrow.The other two dogs emerged asthma after 1 weeks and eating emesia after 2 weeks. Laryngofiberscopy showed that the skin tube had callapsed. The callapsing section almost obstructed the whole lumina. Gross observation showed that the lumina had became narrow.Conclusion1. The concentration of calcium solution can enhance the intensity of calci-um alginate gels .Cartilage cell has become more and more undergrowth.Experimental result showed that the concentration of 300mmol/L calcium solution was the best combining site. In the adequate condition of the calc-ium chloride concentration, Rabbit cartilage cell and human nasoseptal cho-ndrocytes can hold activity and proliferation. Algin is good extracellularma-trix material. But it is easy to deformation in the culture in vitro with cells. It may be on account of the effect of acidoid in the culture and be lack of stimulus of hydrodynamic instability.So, it is essential to improve the equipment of culture in vitro, for example to reform bioreactor. 2. It is exactly to make the cavity block of thyroid cartilage.This study showed that computer technology can configurate exactly according to defective cartilage. It can offer to better repair for defective cartilage. But algin is gelatineous, it is hardly to control solidifiedtime of calcium chloride. So it is diffcult to plastotype in vitro.Alginate gels isn't suitable to plastotype in vitro.It is essential to improve material.3. Experimental study on computer technology aided to construct cartilagin-es tracheales ring withβ-TCP and repair the defect of trachea antetheca showed thatβ-TCP was suitable to plastotype exactly in vitro. According to con-trast experimental group animals and control group animals, it has good clinical practicability.
