The Clinical Effect And Safety Of High Intensity Focused Ultrasound(HIFU) For The Treatment Of Chyluria

Objective: To observe and evaluate the therapeutic effect of high intensity focused ultrasound(HIFU) for the treatment of chyluria .To monitor the volum, structure,bloo -d supply of kidney and renal function after the treatment and estimate the safety of HIFU.Methods: Overall, 42 patients (22 cases with lesion on the left side, 18, on the right and 2, on both ) with chyluria underwent the treatment of HIFU. The diagnosis of chyluria was confirmed by ether test, and the side of chylous reflux was determined by cystoscopy. All patients were treated by BY-01 type high energy focal tumor therapy machine. The heat radiation was adopted in the sick lateral kidney pedicle and upper ureter.Measure the volum of kidney , the inner diameter,peak systolic velocity(PSV) and resistant index(RI) of the renal artery(RA),the inner diameter and velocity of the renal vein,width of the kidney pedicle and upper ureter, Cr and BUN of blood serum before the therapy and 1 week,6 months later check them again.And at the same time, the ether test was done to confirm the curative effect. To analyze the data and judge whether there is significant meaning or not.Meanwhile 30 patients with chyluria undwent retroperitoneoscopic ligation of renal lymphatic vessels or open surgery as the comparison group.To analyze the statistical significance of the therapeutic effect and safety between two methods .Result:1. One week after the therapy of HIFU the ether test of 37 patients turned to negative(88.10%).And 6 months after the therapy,there were 36 negative cases(85.71%).After the ligation of renal lymphatic vessels,29 patients turned to normal(96.67%).There was no significant meaning on therapeutic effect between two methods(p>0.05). 2.After the treatment of HIFU there were no significant change on CDFI and blood serum results(p>0.05).Conclusion: Chyluria could be effectively treated with HIFU. After the treatment,th- ere were no significant injury to the structure and function of kidney. It is safe for the treatment of chyluria via HIFU.
