Object Use ISH and FISH to research the relationship among extranodal Diffuse large B cell lymphoma(DLBCL),Epstein-Barr virus(EBV)and Gene abnormality in Guangxi region,and to investigate the effect and mechanism of EBV in DLBCL.Method①Select 124 DLBCL samples according to HE and Immunohistochemistry(IHC),and then make these samples into tissue microarrays.Use IHC detect CD10,BCL-6,MUM1 dividing DLBVL into two subgroups:germinal center(GCB)type and non-germinal center(non-GCB)type.②Use ISH to detect the expression of EBERs in two subgroups.③Use FISH to detect BCL-6 gene isolation and IgH-BCL2 gene fusion in the two subgroups.④Use IHC to detect Ki-67 and compare the expression of Ki-67 in the two subgroups.⑤Use statistic software to analyse the expression of EBERs in different DLBCL subgroups and analyse the relationship among EBERs,gene abnormality and Ki-67.Result①According to the expression of CD10,BCL-6,MUM1,124 cases are divided into two subgroups:60 cases(48.39%)are GCB type,64 cases (51.61%)are non-GCB type.②In the 124 DLBCL cases the positive expression rate of EBERs is 35.48%,in GCB group 16 cases(26.67%)are positive expression of EBERs,in non-GCB group 28 cases(43.75%)are positive expression of EBERs,the expression of EBERs has statistical differences in two different groups(P=0.047,<0.05)and the infection of EBV is higher in non-GCB group.③In 124 DLBCL cases 36 cases(29.03%)have BCL-6 gene isolation,43 cases(34.68%)have IgH-BCL2 gene fusion.In GCB group the abnormality rate of BCL-6 and IgH-BCL2 is 20%(12/60)and 26.67%(16/60),in non-GCB group the abnormality rate of BCL-6 and IgH-BCL2 is 37.5%(24/64) and 42.19%(27/64).BCL-6 gene isolation has statistical differences between GCB and non-GCB(P=0.032,<0.05),and the abnormality rate is higher in non-GCB group,but IgH-BCL2 gene fusion has no statistical differences in two groups(P=0.07,>0.05).④Spearman rank correlation analysis shows:the infection of EBV and BCL-6 gene isolation have positive correlation in GCB group(P=0.042,<0.05),r=0.264.⑤The expression of Ki-67 in GCB group 24 cases are(+++),22 cases are(++),4 cases are(+),10 cases are(-);in non-aCB group 22 cases are(+++),24 cases are(++),1 case is(+),17 cases are(-).Rank sum test analysis shows:the expression of Ki-67 has no statistical differences in two group(P=0.375,>0.05).Define Ki-67(+++)as the high expression group, (++)(+)(-)as the low expression group,Spearman rank correlation analysis shows:Ki-67 and infection of EBV have no correlation in two subgroups(GCB group P=0.438,non-GCB group P=0.707,>0.05).Conclusion The infection of EBV plays an import role in DLBCL in Guangxi,the infection of EBV in non-GCB group is higher than GCB group, and it is the first time to reveal that in different subgroups DLBCL also has heterogeneity for etiology.The expression of Ki-67 is considered as bad prognosis in many cancers but Ki-67 has no correlation with infection of EBV and different subgroups in DLBCL.These cases which are infected by EBV are easy to occur BCL-6 gene isolation and this relation is stronger in GCB group.
