Background: Lymphoma is a malignant tumor originating from the lymphatic hematopoietic system.EBV is related to a variety of lymphomas.Studies have shown that EBV is mainly transmitted through saliva.After infecting the host cells,it mainly exists in two modes: lytic infection and latent infection.These two infection modes can be carried out alternately.During the latent period of EBV,most of the genes stopped transcription and expressed only part of the proteins,mainly latent membrane proteins(LMP1,LMP2 A and LMP2B),latent nuclear proteins(EBNA1,EBNA2,EBNA3 A,EBNA3B,EBNA3 C,EBNALP),non-coding RNA(EBER1 and EBER2)and 44 mature micro-RNA(mi RNAs).Objective: To explore the expression and characteristics of several EBV associated factors in different types of lymphoma,and to provide a basis for further study of the relationship between EBV latent protein and apoptosis.Methods: 1.EBER was detected by In situ hybridization.2.EBNA2,LMP1 and LMP2A were detected by Immunohistochemistry.3.Interpretation criteria and staining mode:(1)The cells with dark blue staining of intact nuclei were defined as EBER positive cells,and the cases with more than or equal to 10% EBER positive tumor cells in tissues were classified as “EBER positive cases”.What’s more,the cases with less than 10% EBER positive tumor cells in tissues was used as the judgment standard of “EBER detected cases”.(2)LMP1 and LMP2 A criteria: the cells with light brown to dark brown cell membrane or(membrane and plasma)and the cells with dark brown perinuclear dot coloring were judged as positive.(3)EBNA2 positive signals were located in the nucleus and brown.LMP1,LMP2 A,and EBNA2 were not classified as positive or detected cases.Results: 1.Among the 118 cases of lymphoma,63 cases(53.4%)were detected by EBER,and 35 cases(29.7%)were EBER positive cases;LMP1 was detected in 53 cases(44.9%).LMP2 A was detected in 35 cases(29.7%).EBNA2 was positive in only 1 case(0.8%)of Angioimmunoblastic T-cell lymphoma,showing a small number of tumor cells.2.The detection rate of EBER was higher than that of LMP1(P>0.05),but the difference was not statistically significant.The detection rates of EBER and LMP1 were higher than that of LMP2A(P < 0.01),and the difference was statistically significant.3.What’s more,Among the 63 cases of“EBER detected cases”,there were 28 cases showed that EBER positive cells individual or a small number of positive cells scattered distribution,which did not meet the conditions of “EBER positive cases”.It is suggested that the patients were infected with Epstein Barr virus,but the virus may not be the main pathogenic factor.4.According to the order of Diffuse large B cell lymphoma(DLBCL),classical Hodgkin’s lymphoma(CHL),Natural killer/T-cell lymphoma(NK/T),Angioimmunoblastic T-cell lymphoma(AITL),Follicular lymphoma(FL),Mantle cell lymphoma(MCL),Marginal zone lymphoma(MZL)and Peripheral T cell lymphoma(PTCL),the detection rates of EBER were 34.0%,73.7%,100.0%,100.0%,37.5%,83.3%,60.0% and 33.3%respectively.(The EBER detection rates for cases which total number ≤ 2 were not calculated);the rates of “EBER positive cases” were 9.4%,47.4%,100.0%,100.0%,12.5%,16.7%,0.0% and 33.3%;the detection rates of LMP1 were 20.8%,68.4%,100.0%,66.7%,50.0%,33.3%,60.0% and 33.3%,respectively;the detection rates of LMP2 A were 11.3%,68.4%,33.3%,66.7%,37.5%,33.3%,20.0% and 0.0%.The rates of detection and positive for EBER were lower in diffuse large B-cell lymphoma,which were the same as that for LMP1 and LMP2 A.The detection rates of LMP1 in NK / T,CHL and AITL were relatively higher,however,the higher detection rates of LMP2 A were in CHL and AITL.(The detection rates of LMP1 and LMP2 A were not calculated for cases with total number of cases ≤ 2).5.In addition,there were 7 cases(5.9%)were negative for EBER but positive for LMPs,including 2 cases of CHL(NS)and DLBCL(ABC),FL(IIIa),BL,SLL / CLL and PTCL accounted for 1 case respectively.Conclusion: 1.The sensitivity of EBER detected by in situ hybridization was similar to that of LMP1 detected by immunohistochemistry,and both of them were higher than the sensitivity of LMP2 A detected by immunohistochemistry.2.A small number of EBER positive cells were detected in lymphoid tissue,which suggested that EBV infection existed,but this virus may not be the pathogenic factor.3.NK/T,AITL and CHL are closely related to EBV infection,and EBV may be an important pathogenic factor.4.For some lymphoma cases,relying on EBER alone is not enough to negate EBV infection,combined with LMPs detection is helpful to avoid missed diagnosis. |
