| Objective To ascertain the epidemiological characteristics of epilepsy and stroke in Kawa minority group in Yunnan Province first time,and to assist in planning health services for people with epilepsy and stroke in this ethnic minority.Method The Whole study,which surveyed on epilepsy and stroke at the same time in Kawa minority group,with used the way of random cluster sampling and door-to-door survey.The investigation of epilepsy,which was based on the WHO questionnaire and ICBERG screening questionnaire used in China,was fulfilled by applying the latest international diagnostic standard epilepsy.This investigation included the life-time prevalence and the active epilepsy prevalence,incidence,the type of epilepsy,natural remission rate,treatment gap and the situations of epilepsy family.The epidemiological survey on stroke was based on screening questionnaire,all the patients diagnosed as stroke according to the diagnostic criteria of China guideline for Cerebrovascular disease Prevention and Treatment in 2005.The contents of the investigation included prevalence and incidence of stroke,as well as the rate of disability among stroke survivors.Results A total of 10597 people were investigated and 19 people were diagnosed as epilepsy,with the life-time prevalence of epilepsy was1.80‰(2.50‰for males, 1.00‰for females,no difference found between males and females,p>0.05).The prevalence of active epilepsy was 1.51‰.The incidence of epilepsy in the Kawa minority was 18.9/100000per year.The average age of first seizure occurrence was 16.32±12.05 years old,and the average course of epilepsy was 15.95±12.33 years old.Among those patients,89.5%were diagnosed as general tonic clonic seizure and 10.5%as absence seizure.The natural remission rate was 10.5%.The gap of treatment for active epilepsy was 100%.The age peak of seizure was below 30 years old.A definite diagnosis of stroke was made among 10597 people with a prevalence rate207.6/100000(231.7/100000for males,180.5/100000for females,no difference found between males and females,p>0.05).The incidence of stroke was 47.2/100000per year.The average age of first stroke occurrence was 58.20±11.82 years old,and the median course of illness was 3 years.Among those patients,50% were diagnosed as hypertension,the rate of smoking and alcohol use were 86.4%and 72.7%respectively.The rate of disability among stroke survivors was 86.4%.Conclusions The prevalence and the incidence of epilepsy and stroke in the Kawa minority group were lower than mostly other domestic areas.The lower prevalence and incidence of epilepsy and stroke could resulted from some factors such as economic factor,food style in Kawa minority group and lack of diagnosis equipment to detect insidious patients,or the ethnic autogene characteristic and other causes which need further study and research.This investigation showed that almost of patients with epilepsy and stroke did not receive a regular and appropriate treatment.This investigation on epilepsy showed that the proportion of active epilepsy was high.The most common of seizure type was general tonic clonic seizure,which was in accordance with that reported by published literatures.The investigation indicated that certain epilepsy was of characteristic of natural remission.The investigation on stroke found that smoke and alcohol consumption were predominant risk factors in Kawa minority group.The rate of hypertension among the patients was also high.In the respondents,the knowledge of stroke,related risk factors and prevention were extremely poor.This study suggested that,in the ethnic minority of mountainous areas,the local government should formulate reasonable health strategy on epilepsy and stroke. |
