Objective:To study the impact of Shuxuetong treatment on inflammatory cytokines in patients with type 2 diabetic nephropathy, and to evaluate the protect role of Shuxuetong treatment .Methods: 120 individuals were divided to 3 groups. Group 1 (DN)included 40type 2 diabetes mellitus patients with microalbuminuria, and group 2 (DM) included 40 type 2 diabetes without microalbuminuria, while group 3(NC) was normal control group included 40 healthy individuals.Their glycosylated hemoglobin(HbA1-c), urinary albumin (UALB) and urine creatinine for 24 hours, ?2-microglobulin(?2-MG) of serum and urine , serum creatinine(Scr),blood urea nitrogen(BUN), uri-c acid(UA), cholesterin(CHOL), triglyeride(TG) , blood rheology and creactive protein(CRP) were measured before and after treatment.Plasminogen activator inhi-bitor-1(PAI-1) of serum and connective tissue growth factor (CTGF),monocyte chemoattractant protein-1(MCP-1) and interleukin-8 (IL-8) of urine were also det-ected in all those individuals.Results: (1)The serum levels of PAI-1,and the urine level of CTGF,IL-8,MCP-1 were increased by turns in group NC, group DM, and group DN, and the difference was significant. Correlation analysis showed that the levels of PAI-1,CTGF,IL-8,MCP-1 which were positively connected with each other, were positively rel-ated with HbA1c,the course of DM and UALB/CR.(2) The varience of UALB/CR was positively connected with levels of CTGF,?2-microglobulinn(?2-MG) of urine,UALB/CR and negatively connected with GFR in multiple stepwise regression analysis.In the sub-group of GFR<90ml/min, the varience of GFR was negatively connected with levels of IL-8,?2-micro-globulin(?2-MG) of serum,UALB for 24 hours,LDL and the course of DM in multiple stepwise regression analysis.And the affection of IL-8 was most great. (3)The levels of cholesterin(CHOL), triglyeride(TG),blood rheology,CRP and plasminogen activator inhibitor-1(PAI-1) of serum ,connective tissue growth factor (CTGF),monocyte chemoattractant protein-1 (MCP-1),interleukin-8 (IL-8) of urine as well as UALB/ CR significant decreased after treatment in above-mentioned two groups; And interclass analysis showed significant difference between two groups.Conclusion: (1) Inflammatory cytokines such as PAI-1,CTGF,IL-8 and MCP-1 participated the development of DN,and they had coordinated with each other in DN.(2) The process of DNwas significantly associated with the course of DM,UALB,LDL,CTGFand IL-8. The result indicated that the earlier and com-bined treatment was very important for delaying the development of DN.(3) Shuxuetong has the functions of anti-inflammation and renal protection. It can not only decrease cholesterin,triglyeride and improve hypercoagulaba-le state,but also restrain the inflammation process ,lessen the ECM and delay thedevelopment of DN. There was coordination between Shuxuetong and Valsartan.
