BACKGROUND&OBJECTIVE:Hepatocellular carcinoma(HCC) was one of the most common malignant tumors in our country,metastasis and recurrence was it's fundamental feature,and the prognosis was bad.Some studies found that the recurrence was related with portal vein tumor thrombus (PVTT),but the mechanism of PVTT formation was not still clear,abnormal expression of some oncogenes,anti-oncogenes,extracellular matrix metalloproteases,adhesion molecules etc were reported associated with it.In this article,we try to explore the formation of PVTT,analyse the relation between tumor metastasis-suppressor gene KiSS-1 and matrix metalloproteinase-9(MMP-9) with clinicopathological feature of HCC and post-operative recurrence through detecting the expression of tumor metastasis-suppressor gene KiSS-1 and MMP-9 in HCC and PVTT tissus.METHODS:The expression of KiSS-1 and MMP-9 in 50 specimens of HCC and 11 specimens of PVTT was detected by immunohistochemistry(IHC) assay.Followed up post-operative disease-free survival rates at 1-,2- and 3-year,and evaluated it by Kaplain-Meire analysis, log-rank test.RESULTS:1.Expression of tumor metastasis-suppressor gene KiSS-1 and MMP-9 in HCC tissues. KiSS-1 and MMP-9 were mainly expression in cytoplasm of tumor cell.The overall positive rate of the expression of KiSS-1 and MMP-9 were 26%(13/50) and 68%(34/50) respectively.In PVTT group,HCC with PVTT group and HCC without PVTT group,the positive rates of KiSS-1 was 0%(0/11),12.9%(4/31) and 47.4%(9/19) respectively, PVTT group and HCC with PVTT group were both lower than HCC without PVTT group(P<0.05),but there was no significant difference between PVTT group and HCC with PVTT group(P>0.05).The positive rates of MMP-9 in three groups was 81.8%(9/11), 83.9%(26/31) and 42.1%(8/19) respectively,there was no significant difference between PVTT group and HCC with PVTT group(P>0.05),but they were both higher than HCC without PVTT group(P<0.05).2.The relation between KiSS-1 and MMP-9 with clinicopathological feature of HCC.Investigate the relation between the expression of KiSS-1 gene and MMP-9 with the clinicopathological feature of HCC such as Edmonson grade,tumor size,PVTT,HBsAg, hepatic cirrhosis,tumor number,AFP,tumor capsule,TNM stage etc.The expression of KiSS-1 was correlated to TNM stage and PVTT(P<0.01);and MMP-9 was correlated to PVTT and tumor capsule(P<0.01).3.Correlation of KiSS-1 expression to MMP-9 expression.IHC assay showed that the HCC tissues KiSS-1 expression positive were mostly negative with MMP-9 experssion.The expression of KiSS-1 was negatively related to that of MMP-9(r=-0.473,P<0.05).4.Correlation of the expression of KiSS-1 gene and MMP-9 to post-operative disease-free survival rates of HCC.Followed up the situation of 3-year recurrence post opration,50 patients divided into recurrence group and unrecurrence group according whether or not recurrence in two years. The result was 25 patients recurrence in two years,the rate of recurrence was 50%, positive rate of the expression of KiSS-1 gene was 8%(2/25) and 44%(11/25)(P<0.05) in two groups;while MMP-9 positive rate of the expression was 92%(23/25),44%(11/25)(P<0.05) respectively.The disease-free survival rates at 1-,2- and 3-year were significantly higher in KiSS-1-positive group than in KiSS-1-negative group(92.3%,76.9%,61.5%vs.48.6%, 40.5%,35.1%,P<0.05),and the reverse result exits in different expression group of MMP-9(41.2%,32.4%,26.5%vs.100.0%,87.5%,75.0%,P<0.05).The expression of KiSS-1 and MMP-9 were closely correlated with disease-free survival rates of HCC after hepatectomy.CONCLUSION:1.KiSS-1 and MMP-9 are correlated with the PVTT and 2-year postoperative recurrence of HCC closely.2.KiSS-1 can suppress the formation of PVTT and 2-year postoperative recurrence of HCC;MMP-9 can promote the formation of PVTT and 2-year postoperative recurrence of HCC.3.The expression of KiSS-1 is negatively related to that of MMP-9 in HCC tissues. It is speculated that KiSS-1 might suppress the formation of PVTT and 2-year postoperative recurrence of HCC by suppressing MMP-9 expression.4.KiSS-1 and MMP-9 are closely correlated with disease-free survival rates of HCC after hepatectomy,and may be used to estimate the risk of postoperative recurrence of HCC.
