Objective To evaluate the clinical value of diffusion weighted imaging(DWI) in differentiation of benign from malignant prostate leisions.Methods DWI was performed in 4 groups,32 benign prostate hypertrophy (BPH )patients,16 prostatitis patients , 27PCa patients and 30 health volunteers.In the group of health volunteers, 30 males were examined without any symptom of prostate disease. All the PCa patients were proved by pathology.DWI was performed by a 3.0T magnetic resonance unit. DWI were obtained with 3 different diffusion factors,factor b,of 50,400 and 800 s/mm2.The ADC values were measured in each PZ of patient and in each central gland of BPH patient and health one by drawing the regions of interest(ROIs).The ADC values of the bladder were also measured to assess the validitv of the method.Each area was measured 5 times randomly, and the mean value were caculated.Results Acceptable images for ADC measurement were obtained in all the patients. The intensity of PCa appeared to be higher in the DW images and the others appeared to be lower while the b value was graeter.When the factor b was constant,the ADC values of the bladder among 4 groups had no statistical difference,respectively.When b=800 s/mm2,the mean ADC values of all cancer regions, all PZ regions of BPH patients, and that of all inflammation regions and that of all normal ROIs,were(0.82±0.12),(2.16±0.14),(1.38±0.15)and(2.19±0.11)×10-3mm2/s respectively.The mean ADC of PCa was significantly smaller(F=682.98,P=0.00) than that of normal PZ of the prostate, PZ of BPH and the prostatitis.But the difference between the BPH PZ and the normal PZ had no statistical significance(t=0.27,P=0.41). The BPH CG and the normal CG had no statistical significance the same(b=400s/mm2,t=0.43,P=0.34>0.05;b=800s/mm2,t=0.57,P=0.07>0.05). There was a significant statistical difference in the BPH CG, PCa and prostatitis(b=800s/mm2,F=174.13,P=0.00).When ADC<0.84 s/mm2 was set as a diagnostic threshold, the sensitivity,specificity,accuracy and coherence were 84.21%,86.54%,85.92%及0.67.Conclusion ADC value measured from DWI can be used to differentiate prostate diseases.
