Objective To study the correlationship between T cell activating and immune tolerance on chronic hepatitis B(CHB) virus infection by detecting the CD4~+,CD8~+,CD44,CD69 and NKT cell level of peripheral blood mononuclear cells(PBMC). To investigate the characteristics of intrahepatic CD4~+,CD8~+T lymphocytes and K cell in immune tolerant phase of patients with chronic hepatitis B virus infection,and to explore the relationship between intrahepatic immune competent cells and the status of immune tolerance in chronic HBV infection.Method 1.PBMC were isolated from 15 healthy blood donors (as normal controls) , 15 patients in immune tolerant phase and 15 patients in immune active phase by separating medium of lymphocytic cell . The PBMC was stained with anti-CD4 PE,anti-CD8 PE-cy5,anti-CD44 FITC> anti-CD69 FITC and NKT FITC and all these cells analysised with flow cytometry.2.Ten liver biopsies were selected from patients with HBV infection in immune tolerant phase for the immunohistochemical detection of CD4~+,CD8~+ T lymphocytes and K cell subsets,Five normal live biopsies and ten live biopsies of patients in immue active phase were employed to serve as control respectively.Results NKT, CD8 ~+T cell and those activating maker's of CD44,CD69 in PMBC of patients in immune tolerant phase were all significantly increased as compared with those in normal control(t=9.90, 3.63, 12.41, 13.40, all p<0.01),but NKT and CD44,CD69 of the CD8~+T activating maker's are significantly decreased as compared with those in PMBC of patients in immuneactive phase (t=4.83, 7.83, 6.77, all p<0.05). CD4~+ T cell in PMBC of patients in immune tolent and immuneactive phase were all significantly decreased as compared with those in normal control (t=7.45, 3.63, all P<0.05) ,but those activating maker's of CD44,CD69 were all significantly increased(t=12.88 , 9.29, all p<0.01).There was a negative correlation between the activity of T cell and HBVDNA in patients with CHB (r=0.563, P<0.01) .CD4~+ T ,CD8~+T,K cell subsets in the liver biopsies of patients in immune tolerant phase were all significantly increased as compared with those in normal control,but significantly decreased as compared with those in biopsies of patients in immuneactive phase.Conclusion The activity of T cell in PMBC of patients immune tolent were all significantly decreased as compared with those in immuneactive phase,T lymphocytes may play important roles in the clearance of HBV as well as in liver inflammation and injury during chronic HBV infection. The fewer T lymphocyte infiltrations are associated with the status of immune tolerance, it may be one of the mechanisms of CHB's chronicity.
