Objective: Colorectal carcinoma is one of malignant digestive tumors. Its incidence rate and mortality is rising year by year. Many studies show that combined biological effects of multiple genes are involved in the carcinogenesis and metastasis of colorectal carcinoma. The purpose of this study is to investigate the expression of Leptin, Leptin receptor, VEGF and CD34(This specific antibody is use for marking the microvessel desity of tissue to reflect the active level of new vessels in colorectal carcinoma, adjacent carcinoma and normal colorectal tissues to explore their roles in development, invasion and metastasis of colorectal carcinoma and to try to provide referential indexes to the treatment of colorectal carcinoma.Materials and metheds1 68 cases of colorectal carcinoma, 68 cases of adjacent carcinoma and 68 cases of normal colorectal tissues were collected. All the tissues were fixed in 10% neutral formalin and embedded in paraffin.2 The expression of Leptin, Leptin receptor, VEGF and MVD were examined by S-P immunohistochemical stain(IHC) in 68 colorectal carcinoma, adjacent carcinoma and normal colorectal tissues.Results1 The expression consequence of Leptin Expression of Leptin in 68 carcinoma and adjacent carcinoma and normal colorectal tissues were detected by IHC, the positive expression rates were 72.06%(48/68), 42.65% (29/68), 26.47%(18/68) respectively. The expression of Leptin in carcinoma group was higher than that in adjacent carcinoma group and nomal group (P<0.05),but there was no correlation between adjacent carcinoma group and normal colorectal group (P>0.05). Expression of Leptin wasn't correlated with age, sex, tumor position, tumor size, big type of the tumor, histological type(P>0.05), but correlated with pathological grade, the depth of the tumor invaded the bowel wall, condition of the lymph node metastases, Dukes stage, distant place metastasis and canalis haemalis tumor embolus(P<0.05).2 The expression consequence of Ob-RExpression of Ob-R in 68 carcinoma and adjacent carcinoma and normal colorectal tissues were detected by IHC, the positive expression rates were 64.71%(43/68), 33.83% (23/68), 29.41%(20/68), respectively. The expression of Ob-R in carcinoma group was higher than that in adjacent carcinoma group and nomal group (P<0.05), but there was no correlation between adjacent carcinoma group and normal colorectal group (P>0.05). Expression of Ob-R wasn't correlated with age, sex, tumor position, tumor size, big type of the tumor, histological type(P>0.05), but correlated with pathological grade, the depth of the tumor invaded the bowel wall, condition of the lymph node metastases, Dukes stage, distant place metastasis and canalis haemalis tumor embolus(P<0.05).3 The expression consequence of VEGFExpression of VEGF in 68 carcinoma and adjacent carcinoma tissue and normal colorectal tissues were detected by IHC, the positive expression rates were 69.12%(47/68), 41.12% (28/68), 17.65%(12/68) respectively. The expression of VEGF in carcinoma group was higher than that in adjacent carcinoma group and nomal group (P<0.05), and adjacent carcinoma group was higher than that in nomal group (P<0.05). Expression of VEGF wasn't correlated with age, sex, tumor position, tumor size, big type of the tumor, histological type(P>0.05), but correlated with pathological grade, the depth of the tumor invaded the bowel wall, condition of the lymph node metastases, Dukes stage, distant place metastasis and canalis haemalis tumor embolus(P<0.05).4 The relationship of Leptin, Leptin receptor and VEGFThere were 41 cases in the colorectal carcinoma group where the expression of leptin and VEGF was positive simultaneously, the simultaneously negative were 13 cases. Spearman correlation showed that leptin had a remarkably positive correlation with VEGF in colorectal carcinoma (r=0.806, P<0.05). There were 40 cases in the colorectal carcinoma group where the expression of leptin and Ob-R was positive simultaneously, the simultaneously negative were 15 cases. Spearman correlation showed that leptin had a remarkably positive correlation with Ob-R in colorectal carcinoma (r=0.754, P<0.05). There were 40 cases in the colorectal carcinoma group where the expression of Ob-R and VEGF was positive simultaneously, the simultaneously negative were 17 cases. Spearman correlation showed that Ob-R had a remarkably positive correlation with VEGF in colorectal carcinoma (r=0.761, P<0.05).5 The expression consequence of CD34(MVD)MVD value in 68 colorectal carcinoma and adjacent carcinoma and normal colorectal tissues were 21.88±5.87, 13.78±5.32 and 5.54±0.85 respectively, MVD value in carcinoma group was higher than that in adjacent carcinoma group and normal group (P<0.05), MVD value in adjacent carcinoma group was higher than that in normal group (P<0.05). MVD value had a relationship with pathological grade, the depth of the tumor invaded the bowel wall, condition of the lymph node metastases, Dukes stage, distant place metastasis and canalis haemalis tumor embolus(P<0.05).6 The correlation between MVD value and Leptin, Ob-R , VEGFThe MVD value(38.82±8.89) in colorectal carcinoma with postive Leptin expression was significantly higher than that with negative Leptin expression(23.05±7.80), (t=6.774, P<0.05).And with the expression degree increasing, MVD value also increased, correlation analysis showed that MVD value had a positive correlation with leptin (r=0.559, P<0.05).The MVD value(39.90±8.48) in colorectal carcinoma with positive Ob-R expression was significantly higher than that with negative Ob-R expression (24.34±7.91), (t=7.400, P<0.05). And with the expression degree increasing, MVD value also increased, correlation analysis showed that MVD value had a positive correlation with Ob-R (r=0.574, P<0.05).The MVD value(39.41±8.32)in colorectal carcinoma with the positive VEGF expression was significantly higher than that with negative VEGF expression (23.23±8.09), (t=7.469, P<0.05).And with the expression degree increasing, MVD value also increased, correlation analysis showed that MVD value had a positive correlation with VEGF (r=0.590, P<0.05).Conclusion1 The expression of Leptin,Ob-R and VEGF in colorectal carcinoma were significantly higher than that in adjacent carcinoma and nomal colorectal tissues, indicating that Leptin, Ob-R and VEGF were involved in tumorigenesis progression. There was a remarkably correlation between Leptin, Ob-R, VEGF and pathological grade, the depth of the tumor invaded the bowel wall, condition of the lymph node metastases, Dukes stage, distant place metastasis and canalis haemalis tumor embolus, indicating that the high expression of Leptin, Ob-R and VEGF was significantly related with invasion and metastasis of colorectal carcinoma. 2 MVD value in colorectal carcinoma was significantly higher than that in adjacent carcinoma and nomal colorectal tissues, and There was a remarkably correlation between MVD value and pathological grade, the depth of the tumor invaded the bowel wall, condition of the lymph node metastases, Dukes stage, distant place metastasis and canalis haemalis tumor embolus, indicating MVD value was a significant index to measure development, invasion and metastasis of colorectal carcinoma.3 The expression of Leptin, Ob-R, VEGF and MVD value in colorectal carcinoma was consistent, which indicated that Leptin, Ob-R and VEGF might be important factors in angiogenesis of colorectal carcinoma. Meanwhile there were positive correlation between them respectively, which indicated that there were positive cooperation between them in a series of pathological progression (eg:invasion and metastasis) in colorectal carcinoma.
