Objective: Lumbar disc herniation usually accompany with low back pain and / or sciatica. It is one of the main clinical features. The traditional assumes that the nerve root mechanical compression effect may play an important role in the etiology of disc herniation. Further studies'results suggest that the chemical factors, some immunizing factors such as Interleukin 1(IL-1), Tumor Necorsis Factor(TNF) et al also plays an important role. Some author has proposed the autoimmune reaction theory that the autoimmune response is also an important factor which can cause low back pain and sciatica.Inflammation and immune theory also play an important role in the mechanism of pain. In this issue IgG, IgM and macrophage cell's expression levels were observed through immunohistochemistry methods, in the different types of herniated lumbar disc tissue and normal lumbar disc tissue. The patients pre- and postoperative clinical symptoms associated analysis a deeper understanding of patients with lumbar disc herniation in the pathogenesis of autoimmune mechanism, and to explore with local disc macrophage expression levels in patients with lumbar disc herniation and pre- and post-operative symptoms and the relationship between the clinical efficacy. Methods:We collected eight scoliosis patients and 55 lumbar disc herniation patients'disc tissues both in the control group and experimental group. The experimental group and the control group patients were carefully medical examinated. We choose to use JOA score system to evaluate the patients'pain situations. The disc tissue in the experimental group and the control group are obtained during surgery, the groups of specimens with different part of the nucleus pulposus and annulus fibrosus ingredients in the standard immunohistochemical methods for a division test, which is in accordance with the experimental group. The imaging performance in the form of lumbar disc herniated into the annulus fibrosus integrity assessment, annulus rupture persons or extricate himself from the three groups. Observation Group immunohistochemical analysis of the results of each group were of IgG, IgM and CD68 expression condition was analyzed and compared, and with the clinical symptoms and the outstanding form of disc tissue linked to explore their relationship. After 12 days of the pilot group to history-taking and physical examination to record JOA pain score, according to the degree of CD68 expression, immunohistochemical results will be divided into CD68 and CD68 positive-negative group, compared between the two groups of patients, JOA postoperative pain score. Results:The results of the study are as follows: 1.There was the expression of CD68 in each group of the experimental group, in the control group there was no positive expression. From Table 2 it can be seen that there was a significant difference between the experimental group and the control group (P<0.05); The positive rate in Annulus fibrosus integrity lower than the disc rupture group, as well as disc dissociation group with significant difference (P<0.05), 2. Samples of the control group were IgG negative expression, and the experimental group was expressed in each group. From Table 3 we can see that the experimental group of positive rate was significantly higher than that should not one case of positive expression, compared with two notable differences expression of IgG experimental group there has been a significant difference (P<0.05), and with the outstanding level of the increase in the expression of IgG was gradually increasing trend, 3. Samples of the control group were IgM negative expression, and the experimental group was expressed in each group. It can be seen in Table 4: the experimental group were significantly higher than that to the two contrast There was a significant difference (P<0.01), the expression of IgM experimental group there has been a significant difference (P<0.05), and with prominent degrees, and IgM was found gradually increasing trend, the annulus fibrosis a ruptured disc tissue integrity of the annulus fibrosus high rate of positive expression organizations. 4. CD68 positive patients with preoperative pain than CD68 negative group, CD68-positive group than in patients with postoperative pain CD68-light group. From the can be seen in Table 5: CD68 positive patients before JOA pain score lower than the CD68-negative group, compared the two There was a significant difference (P<0.05) from can be seen in Table 5: CD68 positive group after JOA Pain score higher than the CD68-negative group, compared the two There was a significant difference (P <0.05).Conclusions:Macrophages lumbar disc herniation in the immune and inflammatory responses play a major role, and in patients with preoperative clinical symptoms were related, and after disc surgery and the clinical results. Macrophage cells derived from the low back pain patients in the pathogenesis of low back pain plays an important role. Lumbar disc herniation was different forms of IgG and IgM with important influence in the selection. Highlight the extent and degree of annulus fibrosus and tear IgG, IgM is proportional to the output.
