BackgroundSterility is a question widespread throughout the world,in which ovulation failure infertility takes 25%-30%.With modern life and work stress, patients of this disease tend to increase.There has been thousands of years for TCM to treat infertility.Since its concept of determination of treatment based in pathogenesis obtained through differentiation of symptoms and signs, TCM represents its unique curative effect.However,disease diagnosis and efficacy evaluation standards in TCM lack normative and objectivity,which constrains further research and promotion of TCM treatment for infertility. However syndrome theory has provided feasibility guarantee for standardized and objective of TCM differential diagnosis.Cluster analysis has provided a original view and method for standardized and objective research of TCM.ObjectivesThe purpose of this study is to explore distribution patterns of TCM syndromes differential of ovulation failure infertility,to lay the foundation to work out the objective,applicable and workable.standards of syndromes for clinic,then to direct TCM treatment for ovulation failure infertility.MethodsUsing the method of combining literature research,expert consulation, clinical epidemiology investigation with mathematical statistics,to draw up information collection table of four diagnostic methods of TCM syndromes for this disease;and to set up database.Using descriptive analysis,cluster analysis,principal component analysis etc,to make a preliminary study on disposition rule of syndrome differentiation and symptom of TCM.Results1.There are 220 cases consistent with standard.Through differential diagnosis,the experts divided them into 9 patterns.Deficiency of Kidney-yang (42cases)is the most syndrome pattern.The next is Liver-depression(40cases), Kidney-asthenia and hirer-depression(29 cases),deficiency of Kidney-yin(27 cases),phlegm turbidity(27 cases);Kidney-asthenia and blood stasis(6 cases) is less.So we can say the deficiency of kidney is the root of this desease, the phlegm turbidity and stasis of qi-blood is the branch.2.The descriptive analysis of 220 cases' clinical message show:most of the anovulatory infertility patients are graduated from senior middle school or three year' s college,and their jobs are office workers,mental laborers.Psychological factor,such as irritable,depression,anxiety etc., which tend to provoke Liver-depression,takes 85.45%.It shows emotional upset, leading to Lirer-depression,is main etiological factor.Main clinical symptoms are:premenstrual chest distress and restless,premenstrual gas pain of breast,gas pain in chest and hypochondrium,delayed menorrhea,blood clot, menalgia etc.These symptoms are symptoms of Liver-depression.3.220 clinical cases use K-Means cluster analysis for 5 categories: Liver-depression and blood stasis(63cases),deficiency of kidney-yang(54cases),Spleen-asthenia and Liver-depression(53cases), Kidney-asthenia and hiver-depression(27cases),Spleen-asthenia and phlegm tubidity(23cases).Liver-depression and blood stasis pattern is the most pattern.The disease has close relationship with Liver,Spleen,Kidney.143 cases(65.00%),are involved in Liver-depression;and 81cases(36.80%),are involved in Kidney-asthenia.This suggests Kidney-asthenia and Liver-depression were main pathogenesis.There are 103cases(46.80%)of syndromes of mixture of asthenia and stbenia.Conclusion1.Liver-depression and blood stasis is common pattern,ovulation failure infertility has close relationship with Liver,Spleen,Kidney. Kidney-asthenia and Liver-depression are main pathogenesis of anovulatory infertility.Asthenia in the root and sthenia in the branch is the nature of this disease.2.It is reasonable and feasible to usher cluster analysis in research of syndrome differentiation in TCM.
