Expression, Purification And Assay Of Serum Immunology Of Hepatocellularcarcinoma-Associated Antigen KINECTIN

Objective: To investigate the utilizing value of kinectin antibody for HCC in clinic diagnosis, the kinectin antibody in sera of HCC, other tumor, hepatic cirrhosis,and health adult were detectedMethods: The recombinant vector including kinectin gene segment was introducted into E.coli DH5α.The fusion protein, MBP- kinectin, was obtained by means of the expression of genetic engineering and the separation of affinity column. The non-fusion protein, kinectin, was procured after MBP-kinectin was cut off by enzyme.The immunocompetence of kinectin protein and that of MBP-kinectin were compared by means of Western-blot technique. kinectin antibody of the patient's serum of HCC, cirrhosis and other cancer, and some health adult were detected by ELISA. The result was analyzed statistically.Result: (1)In 50 cases of HCC, the antibodies against kinectin of 29 cases was positive (58%), and in 16 cases of the HCC with AFP negative, the one of 7 cases was positive (43.8%). kinectin antibody had complement for the diagnosis of HCC in patients with AFP negative.(2) In 50 cases of liver cirrhosis, the antibodies against kinectin of 13 cases was positive (26%). In 50 cases of gastric carcinoma, the antibodies against kinectin of 9 cases was positive (18%). In 50 cases of colon carcinoma, the antibodies against kinectin of 6 cases was positive (12%). In 50 cases of lung carcinoma, the antibodies against kinectin of 8 cases was positive (16%).In 50 cases of health adult, the antibodies against kinectin of 4 cases was positive (8%).(3) The sensitivity of diagnosing HCC with kinectin was 58%, the specificity of one was 84%, the positive predictive value of one was 42%, the negative predictive value was 91%, the positive likelihood ratio was 3.6, the negative likelihood ratio was 0.5. In our experiment, the sensitivity of diagnosing HCC with AFP was 64.7% (22/34). The sensitivity of diagnosing HCC with AFP and kinectin was 85.2%.Conclusion: the detection of the antibodies against kinectin had the specificity for the diagnosis of HCC. The early diagnosis of HCC with AFP negative play assisting role. The associated detection of AFP and kinectin could raise the early diagnosis ratio of HCC.
