Objective:To study the pathology of liver injury in rats with obstructive jaundice and the relationship between Hepatic injury and Intercellular Adhesion Molecule-1(ICAM-1),Tumor Necrosis factor-α(TNF-α),Interleukin-1(IL-1).Method:Obstructive jaundice in rats can be formed by ligating common bile duct(LCD). And the level of ICAM-1,TNF-α,IL-1 of OJ in serum and their expression in liver tissue, together with serum total bilirubin(TB),alanine transminase(ALT)are observed on the 1st,3rd,7th,14thday.Besides,hepatic histopathology and immunohistochemistry of TNF-αand IL-1 were studied.Results:1)Compared with the rats in the sham-operated group,TB levels in group increased constantly.The Activity of ALT in serum also reached the peak on the 3rdd after ligation and remained of a high level on the 7thd and the 14thd.2)Serum ICAM-1 in the operation groups,which positively related with jaundice degree and time having positively correlaion with lasting time and jaundice degree(P<0.01), was much more higher than that in the sham-operated group.As obstructing lasted,ICAM-1 increased significantly,especially in determination in liver tissue.3)In operation groups,the TNF-αand IL-1 levels in serum and in liver tissue were significantly higher than those in the sham-operated groups(P<0.01andP<0.05) (it's<300μg/100g,P<0.01);both TNF-αand IL-1 showed prominent increasing the 3rdday after operation,and keep a high level from the 7thd to the 14thd.They all have significant positive correlation with ALT change.4)Pathohistological Study in the liver:The structure of Hepatic Lobules of the sham-operated group was normal.On 3rdday after bile duct ligation,new small biliary epithelia cells appeared in the portal areas and around the liver lobulars with some inflammatory cells infiltrated;As epithelial cells of intrahepatic bile duct exfoliated,Some necrosis focuses of hepatic cells were being seen after lw.After 2w,the hyperplastic areas of intrahepatic bile duct epithelial cells enlarged,the cells were fiat and the lumens of the bile ducts were wide,the basal membrane and collagen fibers layers thickened.A few hyperplastic bile duct epithelial cells differentiated into small bile ducts.The hyperplastic bile ducts,fibroblast and collagen fibers divided and reconstructed the original hepatic lobules,so that the residual hepatocyte formed islands-like structures.Microscopic study shows that:The groups after operations showed necrosis and hyperplasia in hepatocyte,together with cholangiectasis and cholestasis in portal areas.And the hepatonecrosis were more obvious as the time passing.Conclusions:1.In obstructive jaundice rats,ICAM-1,TNF-αand IL-1 are all involved in hepatic injury.The express of ICAM-1 in liver tissue has a positive correlation with time and jaundice degree;ICAM-1 and the ALT increased relationally.And that means high ICAM-1 may indicated sever liver injury.ICAM-1 can be used as a important maker in detecting liver injury Caused by OJ.2.Using the ICAM-1 Antiboy may inhibited inflammatory response in liver,which may have some prevention effects on liver injury.This could provide a treatment for liver injury a new possible way.3.TNF-αand IL-1 have significant positive correlation with ALT,which implies that liver damage may be alleviated through eliminating obstruction.It's always good to eliminate obstrution as early as possible.
