Objectives To study the expression of transcriptional factors T - bet/GATA3 mRNA, and the levels of IFN -γ/IL - 4 gene in children with acute idiopathic thrombocytopenic purpura (ITP);to investigate the tendency of polarization of Th1/Th2 in children with ITP from the perspectives of the T - bet/GATA3,hoping to unveil the pathogenic mechanism.Methods 30 patients with acute ITP were chosen in active group,and 28 in the inactive group, while 20 healthy children served as normal controls.T - bet/GATA3 mRNA expression were examined by using RT- PCR method;IFN-γ/IL- 4 levels were measured by using EIASA assay.Results T - bet mRNA expression in children with active ITP(0.40±0.01)was markedly higher than that in children with inactive ITP(0.20±0.01)and healthy controls(0. 11±0.01);and GATA3 mRNA expression in children with active ITP(0.124±0.011)was markedly lower than that in children with inactive ITP(0.170±0.011)and healthy controls (0.181±0.014);significant difference in T- bet/GATA3 mRNA expression was found between inactive children and healthy controls(P<0.01,95%credible interval did not include 1);IFN-γlevels in children with active ITP(269.33±13.03)pg/ml was markedly higher than that in children with inactive ITP(178.84±7.22)pg/ml and healthy controls(149.26±10.66)pg/ml,and IL - 4 levels in children with active ITP(12.34±1.14)pg/ml was markedly lower than that in children with inactive ITP(21.38±1.36)pg/ml and healthy controls (26.41±1.53)pg/ml;significant difference in IFN -γ/IL-4 levels was found between inactive children and healthy controls(P<0.01,95%credible interval did not include 1). Positive correlation was found between levels of IFN -γand T - bet mRNA expression(r = 0. 899,p<0.01)in children with active ITP,while negative correlation with GATA3 mRNA expression was found(r = - 0.823,p<0.01);Positive correlation was found between levels of IL -4 and GATA3 mRNA expression(r =0.781,p<0.01)in children with active ITP,while negative correlation with T - bet mRNA expression was found(r =-0.860,p<0.01).It was interesting that a closer relationship existed between the ratio of T - bet to GATA3 and the ratio of IFN -γto IL -4(r =0.682,p<0.01).Conclusion the high expression of T - bet,IFN -γand the low expression of GATA3,IL-4 indicates the existence of Th1 polarization in acute ITP;T- bet/GATA3 and IFN -γ/IL-4 might be involved in the pathogenesis in children with acute ITP.
