ObjectiveThrough testing helper T cell(Th) related cytokine IFN-γ、IL-4、Bmi-1and IL-17mRNA expression in Children with acute immune thrombocytopenic purpura(ITP),to analyze the functional changes and its clinical significance of Th cells in children with acute ITP, and to discuss the meanings of the related factors in children with acute ITP.MethodsThe experiment patients were divided into the test group and the control group. The children of test group(for36cases of acute ITP) were the inpatient and outpatient who from the First Affiliated Hospital of Xinxiang Medical University from January2012to September2012.All of them were consistent with diagnostic criteria for ITP. Over the same period we select26patients who undergoing elective surgery as control group in ped-surgery.All cases had no history of blood transfusion, vaccination and hormone application in4weeks and the cases of control group had no history of infection in2weeks.There was no significant difference in age and gender (P>0.05) between2groups and we received the informed consent of their families.We choose the RT-PCR method to detect mRNA expression of TFN-γ,IL-4,Bmi-1and IL-17Th related cytokine in peripheral blood lymphocytes; the ratio of changes of T lymphocyte subsets in test group and control group were detected by flow cytometry. Results1> The percentage of CD3+,CD4+and CD8+in the test group were60.54±3.46,30.07±0.79and25.23±2.57,while CD4+/CD8+was1.12±0.19.The percentage of CD3+,CD4+and CD8+in the control group were63.73±4.03,35.41±4.76,21.62±2.53and CD4+/CD8+was1.64±0.28. The levels of CD3+in the test group were lower than that in the control group (t=3.36, P<0.001); CD4+in the test group were lower than that in the control group (t=8.30, P<0.001); CD4+/CD8+in the test group were lower than that in the control group (t=5.49, P<0.001) and the levels of CD8+in the test group was higher than that in the control group (t=8.72, P<0.001)2. The mRNA expression of IFN-γ,IL-4,Bmi-1, IL-17in peripheral blood lymphocytes in the test group were3.84±0.43,1.44±0.39,2.63±0.54,4.69±0.58; The mRNA expression of IFN-γ,IL-4, Bmi-1, IL-17in peripheral blood lymphocytes in the control group were2.880.57,1.87±0.34,3.91±0.92,3.79±0.82; The levels of IFN-γ, IL-17in the test group were higher than that in the control group (t=7.56, P<0.001)(t=5.07, P<0.001) and the levels of IL-4, Bmi-1in the test group were lower.than that in the control group (t=4.52, P<0.001),(t=6.88, P<0.001)。The mRNA expression of peripheral blood lymphocytes was negatively correlated between IFN-γ and IL-4(r=-0.667, P<0.001) in test group, while they were positively correlated between IL-4and Bmi-1(r=0.776, P<0.001) as well as between IFN-γ and IL-17(r=0.712, P<0.001); there were no significant correlation between IFN-γ and Bmi-1(r=-0.206, P>0.05),also between IL-17and Bmi-1(r=-0.214, P>0.05),as well as between IL-17and IL-4(r=-0.220, P>0.05).Conclusions1There was cellular immune disorder in acute ITP children, the proportion of T cell subsets were imbalance,and they might be Th cell dysfunction in children acute ITP;They were abnormal expression of Th1/Th2in acute ITP children, may be an advantage of Th1disease;2Bmi-1decreased may play a role in the pathogenesis of acute ITP;3Th17probably by promoting the secretion of IL-17play a role in the pathogenesis of acute ITP.
