The Expression Of Vascular Cell Adhesion Molecule 1, Vascular Endothelial Growth Factor And Eotaxin In Operative Cavity After Endoscopic Sinus Surgery

Objective To study the expression of vascular cell adhesion molecule- 1(VCAM-1), vascular endothelial growth factor(VEGF)and Eotaxin in hyperplastic tissues after functional endoscopic sinus surgery(FESS)and the roles of them during the restoration of epithelia in operative cavity,and to provide basis for improving the treatment.Methods Use before- after study in the same patient.25 samples were come from 20 patitents with chronic rhinosinusitis with nasal polypsis(CRSwNP),25 hyperplastic tissues in operative cavity were obtained in the 4th to 6th weeks after operation.The control group includes 8 inferior turbinates.Every group was studied with SABC immunohistochemical method and stain HE respectively.Results 1.Abundant eosinophils(EOS)were observed in nasal polyps. The degree of eosinophil infiltration in the hyperplastic tissues were obviously decreased (P＜0.05),but also higher than the control group(P＜0.05),and correlated with the expression of Eotaxin(r= 0.800;P＜0.05).2.VCAM- 1,VEGF and Eotaxin expressed in three areas such as epithelial,submucosal gland and blood vessel cells.The expression of them in CRSwNP and postoperative groups were significant than in control group(P＜0. 05);but no different between CRSwNP and postoperative group in epithelial area(P＞0. 05).In gland and blood vessel cell areas,the expression of them in the preoperative group were stronger than in the postoperative group(P＜0.05).Conclusions 1.The over - expression of VCAM- 1,VEGF and Eotaxin suggested they were involved in pathologic mechanism of CRSwNP.2.The positive correlation of the number of EOS with Eotaxin in hyperplastic tissues indicates that Eotaxin may.promote adhesion and migration of EOS to lead to the recurrence of nasal polyps.3.The positive expression of them in hyperplastic tissues such as "Vesicle tissues" is the manifestation of active epithelia regeneration of the epithelization, is not the real nasal polyp but similar histopathology,and it is phenomena of limited pathological rebuilding in earlier period.So,rational treatment to operative cavity can effectively control the nasal polyps relapse and impel it to normal epithelization.
