Acute leukemia (AL) is a kind of cloning malignant disease of hematopoietic stem cells. Its main therapy is to combine with chemotherapy. The main reason of treatment failure is bleeding, infection and other complications. However, with the emergence of G-CSF and GM-CSF and the rapid development of antibiotics, the numbers of deathfrom the agranulocytosis infection after chemotherapy have dropped markedly. Yet the time for the platelet to recover to the level of normal was usually long .In most clinic instances, the standard preventive measure against chemotherapy-induced thrombocytopenia has been used to transfuse platelet, reduce chemotherapy or delay until marrow recover its hematopoiesis fucntion . However this can often lead to poor outcomes, including reduced disease free periods and overall survival. .Adverse reactions were often seen with several platelet transfusions, such as transfusion reactions, viral and bacterial infections and platelet refractoriness. .Interleukin eleven (IL-11) is a multipotental cell growth factor that directly stimulates the proliferation of hematopoietic stem cells and megakaryocyte progenitor cells. Macrophage. T cell. Epithelium. hepatocytes and induces megakaryocytic maturation resulting in increased platelet production, regulate immune,inhibit inflammation and so on. the Interleukin 11(IL-11) binding to special receptor on surface of cell produced the biological effect by sgp130 triggering pathway of signal transfer.ObjectiveThis study, to evaluate the expression and clinical significance of interleukin11 and soluble glycoprotein 130 (sgp 130) in acute leukemia (AL) patients during primary treatment and to explore the effect of recombinant human interleukin 11(rhIL-11) on the platelet after in treatment patients with AL .MethodsTo investigate the expression of interleukin11 and soluble glycoprotein 130 (sgp 130) in forty-seven first visits with acute leukemia (AL) before treatment and after CR ,the expressions of IL-11 and sgp 130 ALserum specimens were detected by ELISA .The patients in the test group were treated with rhIL-11 (1.5~3mg/d,<=14d).Results The results showed that the expressions of interleukin11 in AL group before treatment were lower than that in the control group significantly (P<0.05). In AL group, after CR, the expression was not significantly different from the control group (P>0.05).What's more, there was positive correlation between the expressions of interleukin11 and the level of the platelet in peripheral blood(r=0.235, (P<0.05), the expressions of sgp 130 in AL group before treatment were higher than that in the control group significantly (P<0.05). In AL group, after CR, the expression was not significantly different from the control group (P>0.05). What's more, the expressions were positive correlated with the level of WBC in peripheral blood(r=0.267, (P<0.05).In the patients treated with rhIL-11, One case was discontinued after the diagnosis of lung pneumonedema and 1 case lost follow-up. In others, the average time for the platelet to recover to the level of 30 x 109/L, 50 x 109/L and100 x 109/L respectively was significant different from not-treated with rhIL-11. the patients treated with rhIL-11showed higher complete response rate than that not-treated, but there was not significant difference (P>0.05).ConclusionInterleukin11 and soluble glycoprotein 130 (sgp 130) were correlated with acute leukemia, so to investigate the level of interleukin11 and soluble glycoprotein 130 (sgp 130) in patient with acute leukemia (AL) is useful to understand the patients' condition, evaluate efficacy, propose treatment and judge the prognosis. Recombinant human interleukin (IL)-11, can prevent chemotherapy -induced thrombocytopenia and accelerate platelet recovery is little side effect and is a safe and effective drug.
