Meta-analysis On Low Molecular Weight Heparin To Unstable Angina Pectoris Of Chineses

Objective:To synthetically evaluate clinical effect of Low Molecular Weight Heparin (LMWH) in the treatment of Unstable Angina Pectoris(UAP) of Chinese.Methods:Search literatures related with the clinical efficacy of LMWH in UAP which have been made public from 1999 to 2007 in CNKI,VIP and Wanfang Database.Determine the inclusion and exclusion criteria,and extract the data messages after evaluated the quality assessment of included literatures.Revman 4.2 software was used for mata-analysis.According to the result of Heterogeneity test we decided which model would be used,fixed effects model or random effects model. Sensitivity analysis and publication bias evaluation were carried out.Results:(1) Through computer and manually inspection of relevant magazines,there are 298 papers are obtained finally.According to the including and excluding standards,there are 13 randomized controlled trails involved in this study.On the basis of Jadad scale,eight papers are high-quality studies,while the others are low-quality stydies.(2) The targets type of this study is two sort variable,so OR is selected as pooled variable. According to the result of heterogeneity test,13 recearches which have homogeneity are carried on by fixed effects model(Peto method) combining variables and the test of combining impact variables are made through forest plot.(3) The result shows that in PP analysis OR(fixed) 95%CI 5.94[4.58,7.72]and overeffect Z=13.37(P＜0.00001),suggesting in treatment of UAP,LMWH has a significant difference with conventional treatment of coronary heart disease.The 95%CI of combined OR landed to the right of invalid axis in forest plot suggested that the efficacy of LMWH in treatment of UAP is reliable.(4) In sensitiveness analysis,we get OR(fixed)95%CI 6.05[4.61,7.96]through excluding the study which dose not use blind method.By exchanging fixed effects model and random effects model and amount of effect varied(OR and RR),we get RR(fixed) 95%CI3.65[3.52,3.79],RR(random) 95%CI 3.56[3.38,3.75],OR(fixed)95%CI 5.94[4.58,7.72],OR(random)95%CI 7.31[5.32,10.05],which shows slight diversities of all the results.It proves that the analysis result is relatively steady. (5) The funnel plot of this study show a little asymmetric,which demonstrate that there is potential publication bias of the included literatures.Conclusion:According to Meta-analysis,Conventional treatment of coronary heart disease joint with Fraxiparine is superior to conventional therapy in improving patients' symptoms, decreasing patients' level of AUP,and improving the quality of life,which suggests that LMWH is in effect on treating UAP of Chinese.
