PartⅠ:Comparison of efficacy to orotracheal intubation with TruviewTM EVO2 laryngoscope and Macintosh laryngoscope in obese patientsObjective To compare the efficacy to orotracheal intubation with TruviewTM EVO2 laryngoscope and Macintosh laryngoscope in obese patients and evaluate the value of TruviewTM EVO2 laryngoscope for managing the difficult airway situations.Methods Eighty obese patients,ASA physical statusⅠ-Ⅱ,aged 18-65 years,scheduled for elective surgery under general anesthesia requiring orotracheal intubation,were randomly divided into two groups(n=40 each):T group and M group.Information was collected identifying the patient demographic and airway assessment characteristics.After intravenous anesthetic induction,orotracheal intubation was performed.The patients in T group were given a Cormack-Lehane grade of laryngeal view using Macintosh laryngoscope first,then the patient's glottis was exposed by TruviewTM EVO2 laryngoscope,after given the Cormack-Lehane grade of laryngeal view,the patient was intubated.The patients in M group were given a Cormack-Lehane grade of laryngeal view using TruviewTM EVO2 laryngoscope first,then the patient's glottis was exposed by Macintosh laryngoscope,after given the Cormack- Lehane grade of laryngeal view,the patient was intubated.Parameters were recorded,including the laryngeal exposuring and intubation time,intubation attempt, number of fail and tum to FOB,trauma to the airway and complications correlated with tracheal intubalion,such as pharyngalgia post operation,hoarseness.Statistical analysis was performed using the Statistical Package for Social Sciences(SPSS) Version 13.0.A P value of less than 0.05 was considered statistically significant.Results There was no significant difference in the patient demographic and airway assessment characteristics between the two groups(P>0.05).The TruviewTM EVO2 laryngoscope achieved better Cormack-Lehane grade of laryngeal view than Macintosh laryngoscope did in obese patients(P<0.05).There was no significant difference in the laryngeal exposuring time between the two groups(P>0.05).The intubation time in T group was significantly longer than those in M group(P<0.05),but no more than 60 seconds.The achievement ratio at first intubation attempt in T group was significantly higher than those in M group(P<0.05).There was no significant difference in the trauma to airway and complications correlated with orotracheal intubalion between the two groups (P>0.05).Conclusion In obese patients,TruviewTM EVO2 laryngoscope yielded superior efficacy to orotracheal intubalion compared with Macintosh laryngoscope,TruviewTM EVO2 laryngoscope possibly has some utility in managing some difficult airway situations. PartⅡ:Comparison of hemodynamic responses to orotracheal intubation with TruviewTM EVO2 laryngoscope and Macintosh laryngoscope in obese patientsObjective To compare the hemodynamic responses to orotracheal intubalion with TruviewTM EVO2 laryngoscope and Macintosh laryngoscope in obese patients.Methods Forty obese patients,ASA physical statusⅠ-Ⅱ,aged 18-65 years,scheduled for elective surgery under general anesthesia requiring orotracheal intubation,were randomly divided into two groups(n=20 each):T group and M group.Information was collected identifying the patient demographic.After standard intravenous anesthetic induction, orotracheal intubation was performed with TruviewTM EVO2 laryngoscope or Macintosh laryngoscope,respectively.Noninvasive HR,SBP,DBP,MAP and SpO2 were recorded before(T0) and after anesthetic induction(T1),at intubation(T2),1minute(T3),3 minute(T4) and 5 minute(T5) after intubation.The laryngeal exposuring and intubation time were also noted.Statistical analysis was performed using the Statistical Package for Social Sciences (SPSS) Version 13.0.A P value of less than 0.05 was considered statistically significant.Results There was no significant difference in the patient demographic between the two groups(P>0.05).Except for HR at intubation(T2)(P<0.05),there was no significant difference in the hemodynamic data at any other time point of the oberservation between the two groups(P>0.05).There was no significant difference in the laryngeal exposuring time between the two groups(P>0.05).The intubation time in T group was significantly longer than those in M group(P<0.05),but no more than 60 seconds.Conclusion The hemodynamic responses produced by orotracheal intubation using TruviewTM EVO2 laryngoscope and Macintosh laryngoscope in obese patients are similar.
