The Relationship Between Smad4, CyclinD1, P21waf1 Protein Expression And Clinicopathology Features In Gastric Carcinoma As Well As Patient Survival

Objective To evaluate the expression of Smad4,CyclinD1,p21waflprotein and MVD, MLD in gastric carcinoma,cancer-adjacent tissue,lymph nodes and determine the relationship between them and tumor clinicopathological features as well as patient survival.Materials and methods 200 surgically resected gastric carcinoma samples during 1999-2004,56 cancer-adjacent tissues,55 lymph nodes and 30 normal gastric mucosa samples were collected.Tissue microarrays was used to determine Smad4,CyclinD1,p21waflprotein expression and MVD,MLD by immunohistochemistry S-P(Streptavidin-Peroxidase).Software SPSS15.0 was used to analyse data.Results 1 Positive rate of Smad4 in 200 gastric carcinoma tissues is 67.0% (134/200),CyclinD1 and p21waflare 56.5%(113/200),59.5%(119/200),respectively. No correlation between Smad4 and cyclinD1 by spearman's rank correlation analysis in gastric carcinoma(r=-0.092,P=0.195),while significant correlation were observed in Smad4 and p21waflexpression(r=0.179,P<0.05);there is no relationship between CyclinD1 and p21waflexpression(r=-0.046,P=0.518).No correlation between Smad4,p21wafland MVD/MLD,CyclinD1 expression associates with MLD,but not MVD.MVD relates with MLD(P<0.01).2 Smad4 expression is found to be statistically associated with tumor different -iation status and Lauren's types(P<0.01),and CyclinD1 is associated with age, tumor differentiation status and Lauren's types(P<0.05),while p21waflassociated with tumor differentiation status,Lauren's types and lymph node metastasis(P<0.01).MVD is relates with tumor differentiation status and the depth of tumor invasion(P<0.05);MLD affects tumor sizes,differentiation,lymph node metastasis and Lauren's types(P<0.05).3 Positive expression of Smad4 is related with prognostic survival analysed by Kaplan-Meier and Log-rank test(χ2=4.090,P=0.043),but not an independent prognostic factor.Lymph node metastasis and the depth of invasion are independent prognostic factors.Conclusions 1 Smad4 expression rate reduces in gastric carcinoma,and its abnormal expression is related with differentiation and Lauren's types,this indicates that expression of Smad4 influence tumor differentiation;p21waflexprssion reduces in gastric carcinoma,too,and which is correlates with differentiation, Lauren's types and the age.Its abnormal expression may rezult in tumorigenesis; CyclinD1 appears to be overexpressed in gastric carcinoma,and it is relates with lymph node metastasis,differentiation and Lauren's types.MVD/MLD level rises in gastric carcinoma,and the level,which is related with the bio-behaviors of gastric carcinoma closely,affects the tumorigenesis and development of gastric carcinoma greatly.2 Smad4 is correlates with p21wafl,which indicates that reduced expression of smad4 leads to abnormal expression of its downstream gene p21wafl.MVD correlates with MLD and,they interact in gastric carcinoma.3 Survival rate in positive expression of Smad4 surpasses the negative express -ion.Positive expression of Smad4 is a favorable prognostic factor,but not an independent one in patients with gastric carcinoma.
