Objective: To study the effect of ischemic preconditioning on against ischemia/reperfusion injury of residual liver in rats . to investigate the effect of ischemic preconditioning on regeneration of residual liver after reperfusion injury in rats by comparing CyclinD1 expression in three group.Methods: 144 Wistar rats were randomly divided into group ischemia /reperfusion (IR), group ischemic preconditioning (IP) and group partial hepatectomy(PH).Specimen was harvested at 0,1,3,6,12,24,48and 72 hour respectively in three group.Serum ALT and AST were detected by b-iochemery autoanalyzer, liver tissues were stained with liver tissues hem-atoxylin and eosin (HE) for histologic study and CyclinD1 were detected by immunohistochemistry to study its expression changes in three group.Results:(1)In three groups, the serum ALT and AST increased gradually after partial hepatectomy. The peak was at 24 hour ; Compared with IR group , the serum ALT in IP and PH group were lower significantly than those in IR groups at check time point., the serum AST also decreased signifyca-ntly in IP and PH group(P<0.05).(2) Hepatic necrosis and the inflammatory cells infiltration aggravated in IR groups along with the reperfusion. Compared with IR group, IP and PH group showed less hepatocyte injury and smaller necrotic area in liver.(3) CyclinD1 increased slightly after partial hepatectomy and increased Significantly at 12 hours after partial hepatectomy , the high level was kept to 72 hours after partial hepatectomy, its peak time occurred at 24 hours after partial hepatectomy. Compared with IR group, the expression of CyclinD1 in PH and IP group was significantly higher at 12,24,48 and 72 hours after operation (p<0.05).(4) Hepatic regeneration rate increased gradually at 24 hour after partial hepatictomy. Compared with PH and IP group, HRR in IR group were lower significantly at 24,48 and 72 hours after operation (P<0.05).(5) Mitotic Index increased gradually at 24 hour after partial hepatictomy. Compared with PH and IP group, MI in IR group were lower significantly at 24,48 and 72 hours after operation (P<0.05). Conclusions:1. ischemic preconditioning protected the residual liver from warm ischemia/reperfusion injury after rat partial hepatectomy.2. ischemic preconditioning promoted the CyclinD1 expression of residual liver , promoted cell to enter cell cycle and promoted cell proliferation in the early reperfusion.3. Protective effect of ischemic preconditioning on warm ischemia /reperfusion injury of residual liver after rat parial hepatectomy has a instructive significance for protection of hepatic function in hepatic surgery especially for protection of donor's residual liver in LDLT,and sho- uld be investigated extensively.
