Objectives:To investigate the impacts of ischemic preconditioning on regeneration and function of remnant liver with ischemia reperfusion injury after partial hepatectomy in rats and to observe the NF-κB level during the course,thereby to provide a useful basis for clinical appliance.Materials and methods:144 Male Sprague-Dawley(SD)rats were subjected to 30min of partial hepatic ischemia,and 70%partial hepatectomy were performed just before reperfusion.Animals were pre-treated with either IPC(10/10 min,IPC group),or IPC + PH(IPC + PH group),or not pretreated(PH group).The serum AST and ALT levels, hepatocyte proliferation,histological changes and NF-κB levels of the remnant liver were determined in IPC and IPC + PH groups and compared with PH group to observe the impacts of IR and IPC on hepatic regeneration and liver function.Results:IPC ameliorated hepatic injury significantly reflected by liver histology and serum AST and ALT levels.Rats subjected to 70% hepatectomy and 30 min of hepatic ischemia showed significantly decreased hepatocyte proliferation(mitotic index,proliferating cell nuclear antigen,and relative liver weight)when compared with animals subjected to hepatectomy alone(P<0.05).However,hepatocyte proliferation was markedly increased in rats pretreated with IPC when compared with ischemic control group(P<0.05).There was an significant decrease in NF-κB level in IR+PH group after hepatectomy(P<0.05),and the peak was about 1-3hours delayed.No significant difference in NF-κB levels was observed between PH and IPC group of animals at any reperfusion period.Conclusion:Ischemic preconditioning ameliorates the hepatic injury associated with ischemia-reperfusion and has a stimulatory effect on liver cell regeneration that may make it valuable as a hepatoprotective modality. NF-κB appears to be an important mediator in promoting regeneration after combined ischemia and hepatic resection.
