Nurses' Compliance To Standard Precaution And Its Influential Factors

Objective:To evaluate compliance to Standard Precaution in registered nurses;To investigate the related factors,and provide reference evidence for correlated department to institute policy for preventing nosocomial infections and for professional group to formulate training programs or items,so as to promote nosocomial infections prevention.Methods:It was a cross-sectional study with samples of 500 registered nurses from 12 hospitals in ChangSha and Yueyang during June 2007 and September 2007.Five instruments were used to collect information of subjects′demographic characteristics;knowledge,health belief about Standard Precaution;general self-efficacy;and compliance to Standard Precaution.Descriptive analysis,ANOVA,non-parameter H test and multiple linear stepwise regressions and Spearman correlation analysis were performed by the Statistical Package for Social Science(SPSS)13.0.Results:444 valid questionnaires were collected.(1)The average score of Standard Precaution compliance of registered nurses was 51.73±12.62,score ratio was 64.66%,Among them,score of washing hands and hands disinfection items were the highest(3.42±0.50), score of appropriate treatment to sharp utensil and wound(3.08±0.70)was the second highest,and the score of personal protect equipment useage was the lowest(1.69±1.23),glove useage(2.55±0.84)was the second lowest.(2)The average score of knowledge and the health beliefs about Standard Precaution of registered nurses was 16.96±1.86,81.99±7.46 respectively and the score ratio of them was 83.47%,74.54%respectively; and the average score of general self-efficacy was 2.61±0.51.The knowledge,health beliefs about Standard Precaution and general self-efficacy were positive correlation with Standard Precaution compliance, significantly influenced compliance to Standard Precaution.(coefficient correlation was 0.237,0.202 and 0.206,all P<0.001).(3)According to multiple linear stepwise regression analysis,ten factors (grade of hospital;training;department;sharp utensil collector;knowledge; health beliefs about Standard Precaution,perceived barrier and clue of action, severity of occupational infection;general self-efficacy and contact exposed experience)were the predict factors of compliance to standard precaution of the subjects′,explaining 35.7%of its changes.But subjects′age,work time, education degree,professional title and administration duty did not significantly influence compliance to Standard Precaution(all P>0.05).Conclusion:(1)The compliance to Standard Precaution in registered nurses was low, especially at personal protect equipment useage items.(2)This research showed that hospital grade,training,department and sharp utensil collector;knowledge;health beliefs about Standard Precaution, perceived barrier and clue of action,severity of occupational infection; general self-efficacy and contact exposed experience were the main factors which influenced the nurse′s Standard Precaution compliance.(3)In order to avoid or reduce the hospital infection and protect the health of patients and medical members,relevant authorities and hospital infection control department should pay more attention to the nurse′s Standard Precaution compliance,strengthen the Standard Precaution training, provide sufficient pragmatic personal protect equipment.
