Objective: The paper bases on epidemic investigation of essential hypertension(EH) in Kazakh people in Shawan region in Xinjiang. It detects the relationship between the angiotensin-converting enzyme gene insertion/deletion(AT1R A/C) polymorphism, the perilous environmental factors and the outbreak of essential hypertension(EH) in Kazakh people. It also explores the influence of reciprocity of AT1R A/C and environmental factors on essential hypertension(EH) in Kazakh people.Methods: After the investigation of the present situation of essential hypertension(EH) in Kazakh people in Shawan region, 200 essential hypertension(EH) pacients were chosen as case group and 220 people with normal blood pressure were chosen as case-control group. Research was done on risk fectors using case-control method in order to filter pathogenic fectors related to essential hypertension(EH) of Kazakh people. Then prevalence rate of hypertension in Kazakh were counted and analysis were made on the environmental factors to hypertension in Kazakh. Based on the cross-sectional study , sampling 151 cases of the hypertension patients as the control, 200 examples with the normal blood pressure as the matched control, and carring on polymerase chain reaction (PCR) method to detect its genotypes, we analysed the relationship between angiotensin-converting enzyme gene insertion/deletion polymorphizm and hypertension , exploresd the interaction of angiotensin-converting enzyme gene insertion/deletion polymorphism- environment on essential hypertension in Kazakh Population of Xinjiang.Statistical analysises adoptχ2 test,one-factor ,multivariate analysis,count the values ofχ2 ,P,OR,OR95% CI and check the accordance with Hardy-Weinberg equilibrium in the group. Results: 1. Analysis of nvironmental risk fectors:1) Analysis of variancethe showed that despit the descrepency of age and agenda, tast, obesity, overweight, drinking and so on are related to hypertension essential hypertension 2) The screening of unconditional logistic regression analysis showed that large intake of salt(OR=3.79,95%CI:2.33~6.16), large intake of tpreserved meat (OR=2.44,95%CI:1.19~5.03), the obesity(OR=5.07,95%CI:2.72~9.46), the overweight (OR=2.08 ,95%CI:1.22~3.55) and so on are related to essential hypertension. 3) It is showed that weight-control, reduse the intake of salt and preserved meat can redu the risk of essential hypertension of Kazakh people. 2. Interaction of the AT1R A/C polymorphism with environmental factors on EH 1)Among cases with hypertension,the frenquency of Angiotension-converting enzyme A/C genotype were78.5,21.5;compared with controls'81.6%,18.4%respectively,the two groups have no significant difference (χ2=0.537,P≥0.0≥). 2) Angiotension-converting enzyme AC genotype might interact with eating excessive salt, eating salty flesh chronically and overweight, the values of ORs are4.77(2.15~10.15),6.96(2.33~20.76),4.72(1.52~14.66),2.67(1.11~6. 42)respectively. Conclusions: 1) Despit the factors of agenda and age, large intake of salt, traitional milk-tea , obesity overweight, eating preserved meat more than half a year are the key factors of inducing Essential Hypertension of Kazakh people in Xinjiang Shawan region. The research shows that adjust of diet and losing weight will reduce the risk of Essential Hypertension of Kazakh people. 2)Interaction exist between obesity, intake of large amount of salt and family history of Essential Hypertension. Obesity, intake of large amount of salt and family history of Essential Hypertension will increase the risk of Essential Hypertension. 3) There is no relationship between the angiotensin-converting enzyme gene (A1166C) polymorphism and essential hypertension of Kazakh people in Xinjiang Shawan region. 4)If the Kazakh people overweight and drinking 10 bowls traitional milk-tea per day, the AC genetype is more dangerous with Essential Hypertension than AA genetype. 5)There is no interaction between angiotensin-converting enzyme gene (A/C) polymorphism and ACE (I/D) polymorphism of essential hypertension of Kazakh people.
