Objective: To observe the changes of the oxygen free radical (OFR) and antioxygen level that existed in cavernous body of penis of diabetus mellitus (DM) rat with erection dysfunction (ED); to observe the morphological change under the electron microscope to probe into the pathogenesis of DMED.Methods: Rats were divided into the normal control group (n=15) and DM model group (n=60), randomly. The DM model group were injected with streptozotocin (STZ) after fasting twelve hours, sixteen rats were the drug control (STZ group) that had failed to model, 35 rats were DM group that had successed to model. All rats were bred for 12 weeks regularly, 17 ED rats were selected by apomorphine in DM group. The level of malondialdehyde (MAD), superoxide dismutase (SOD), glutathione (GSH), glutathione peroxidase (GSH-px) of cavernous body of penis were determined respectively after anesthetizing, and to observe the change of its ultramicrostructure. Results: The level of MDA of the normal control group, STZ group, DM group and DMED group were 3.138±0.639 , 3.666±0.581 ,4.912±0.881, 11.716±2.712 nmol/mgprot on the level of antioxygen, compared with the normal control group, STZ group, DM group, DMED group had significant difference (χ2=47.517, P<0.001). The normal control group had no significant difference compared with STZ group. The SOD, GSH and GSH-px on the antioxygen level of cavernous body between the normal control group, STZ group, DM group and DMED group were 103.324±10.791, 98.813±14.236, 95.934±14.235, 61.180±18.798 U/mgprot, and 48.4960±3.482, 46.0307±4.537, 34.0667±3.613, 24.3727±2.043 mg/gprot, and 307.888±18.833, 281.290±24.342, 266.829±30.555, 174.396±23.924 U/mgprot, respectively. The DMED groups had significant difference compared with the normal control group, STZ group and DM group (SOD: F=23.99, P<0.001; GSH: F=150.42, P<0.001; GSH-px: F=82.33, P<0.001), The normal control group had no significant difference compared with STZ group.Under the electron microscope, the smooth muscle and blood vessel endothelium of cavernous body were roughed and breakaged, blood capillary were thrombogenesised, the mitochondria swelled and distend, the desmocyte activated, the nerve fiber medullary sheath degenerated, all those were observed in the DMED group.The similar change not observed in the normal control group and STZ group.Some similar change had observed in the DM group .Conclusion: The level of MAD increased and its activity of antioxygen enhanced, the SOD, GSH, and GSH-px and its antioxygen activity decreased in cavernous body of penis of DM Rats, so it resulted the change of ultramicrostructure. The smooth muscle and blood vessel endothelium of cavernous body were roughed and breakaged, blood capillary were thrombogenesised, the mitochondria swelled and distend, the desmocyte activated, the nerve fiber medullary sheath degenerated, all of the one of the causes of DMED. The DM was aggravated, the OFR increased and the activity of cleaning decreased, the change of ultramicrostructure manifested the followed by the lasting of course, so DM rats made to ED more and more.
