Cross-talk Between Insulin Resistance And Angiotensin â…¡ Involve In The Development Of Coronary Arteriosclerosis

Objective: To explore the relationship among the serum insulin, angiotensinâ…¡and coronary lesions,it played the role in the prevention and treatment of CHD.METHODS: In the study, 100 cases suspected with Coronary Heart Disease (CHD)(M53/F47, 42-79years) were selectd in the department of cardiology in The first affiliated hospital of nanchang college from July 2007 to December 2007. According to the result of coronary angiography and the American Diabetes Association's(ADA)diagnotic standard of diabetes mellitus in 1997, the patients were divided into 3 groups: normal control group (n=26), simple CHD group (n=21), CHD accompanied with impaired glucose tolerance or type 2 diabetes mellitus(T2DM) group(n=53). All candidates were measured respectively fasting blood glucose(FBG), total cholesterol(TC), triglyceride(TG), high density lipoprotein (HDL), low-density lipoprotein(LDL) fasting insulin(FINS), insulin resistance index(IRI),angiotensinâ…¡(Angâ…¡). All cases had coronary angiography, measured the extent of coronary narrow degree. According to Califf's jeopardy score, six coronary arteries were analyzed by total amount integral method.RESULT:â‘ The levels of FINS,IRI, Angâ…¡in CHD accompanied with T2DM or IGT group were significantly higher than those of simple CHD group and normal control group ( P <0.01).The levels of IRI,Angâ…¡in simple CHD group were higher than those of normal control group (P <0.05).â‘¡Grouped by coronary artery diseases, the levels of FINS, IRI,Angâ…¡were significantly higher than those of normal control group (P<0.01).â‘¢Correlation analysis showed that the levels of IRI,FINS were positively correlated with those of Angâ…¡( P < 0.05),and the levels of FINS,IRI,Angâ…¡were positively correlated with the jeopardy scores ( P < 0.05).CONCLUCIONâ‘ The levels of FINS,IRI,Angâ…¡in CHD patients are high.The levels of IRI and FINS were positively correlated with those of Angâ…¡. â‘¡Insulin resistance might occurs in all CHD patients. Jeopardy scores were positively correlated with the levels of FINS,IRI and Angâ…¡.â‘¢The degree of insulin resistance correlates with the levels of Angâ…¡. Interaction between IR and Angâ…¡may involve in the development of CHD.â‘£Angiography results combined with IRI,FINS and Angâ…¡might better described the lesion of coronary artery, and might better evaluated the prognosis of patients with CHD.
