Polycystic Ovarian Syndrome(Polycystic Ovary Syndrome PCOS) is a complex pathogenic clinical manifestation polymorphism.The incidence rate among women of reproductive age is about 6%,The cause of PCOS is still to be found,PCOS's complicated biochemical characters such as hyperandrogenemia and insulin resist coming from ovary and adrenal gland can also show that various genes may involve in.With the development of biography technology and constant penetration of genitourinary research, people have realized that the mechanism of causing PCOS has something to do with various genes except for the unbalanced regulation of hypothalamic-pituitary-ovarian.This research is aiming at observing PCOS patients' blood serum(insulin-like growth factor-1)IGF-1 and Leptin are thick with its cause mechanism.IGF-1 is also named Somatomedin which is a single chain-basic polypeptide consisting of 70 amino acid residues.It has molecular weight of 7649Da and is coded by the gene of the 12th chromosome.IGF-1 plays an important role of regulating ovarian function through autocrine and paracrine.Excessive IGF-1 will stimulate follicular development over-mixture between androgen and estrogen.Leptin is coded by OB gene, protein hormone secreted by lipocyte is made of 167 amino acids. High-level Leptin can inhibit the aromatization of granular cell androstendione and stop the transform from androgen to estrogen,in order to enhance the serum androgen standard.Parts of P COS patients' bodies, especially the fat ones,are gave birth to synalbumin mechanism from organism,which results in pancreas cells insulin increase and hyperinsulinemia.The biological features of IGF-1 and Leptin root the close relationship with PCOS.This research collected 47 patient examples who were hospitalized from Feb.2006 to Nov.2007.Based on the diagnostic standard of PCOS,it divided them into 2 groups:1.Normal contrast group 22 ones:2.PCOS group 25ones.And being divided again,they become A,B,C,D 4 groups:A Non-fat normal group 12 ones:B Fat normal group 10 ones:C Non-fat PCOS group 13 ones:D Fat PCOS group 12 ones.Testers are all aging from 19 to 34, 27.18±3.15 on average.The PCOS diagnostic standard is;(1) Clinical manifestation:menstruation change such as lack menstrual flow,oligomenorrhea,amenorrhea,dysfunctional uterine bleeding as well as anovulation,sterility and corpus luteum incompetence.(2) Biochemical character:androgen level is twice beyond the normal upper lim itation,LH is high whereas FSHis low,LH /FSH≥2~3.(3) Type-B ultrasonic:both of the ovaries are enlarged in volume which diameter is about 2~8mm,and the mesenchymal echo is strengthened. Diseases of adrenal gland and thyroid gland are excluded as well as Cushing syndrome.With choosing 22 patients from contrast group at the same time requiring regular menstrual cycle,ovarian tumor and hysteromyoma and endometriosis uterine and abnormal metrorrhagia and so on are excluded by type-B ultrasonic,incretion inspection and laparoscope examination. With elisa,test the IGF-1 and Leptin.By SPSS13.0 software processing system,conduct the t test and straight line correlation analysis for data identified by X±S.This research selected 22 normal pregnant women as well as 25 PCOS pregnant women,and got the contrast group serum IGF-1 average level is 301.81±40.02ng/L;that of PCOS group is 360.43±70.75 ng/L.PCOS group's IGF-llevel is more higher than that of contrast group. The difference has remarkable significance(P<0.01),which shows that within PCOS,IGF-1 secretion regulates abnormally.High IGF-1 is also an incretion character of PCOS patients.IGF has positive correlation with limosis insulin level,which indicates that high IGF has something to do with PCOS hyperinsulinemia.And IGF-1 has positive correlation with androgen,which indicates that IGF-1 has something to do with PCOS hyperandrogenemia.In the analysis of IGF-1 and BMI,IGF-1 level is only affected by group factor,unrelated to BMI factor(P<0.01=;the average level of contrast serum leptin is8.41±3.69ng/L;PCOS group is22.05±5.87ng/L.PCOS group's leptin level is more higher than that of contrast group.The difference has remarkable significance(P<0.01),which shows that within PCOS,Leptin secretion regulates abnormally.High Leptin is also an incretion character of PCOS patients.Leptin has positive correlation with limosis insulin level,which indicates that high Leptin has something to do with PCOS hyperinsulinemia.And leptin has positive correlation with androgen,which indicates that leptin has something to do with PCOS hyperandrogenemia.In the analysis of leptin and BMI,non-fat group PCOS patients' leptin level has nothing to do with BMI.However, fat group PCOS patients' leptin level has positive correlation with BMI, which indicates that overweighed or fat PCOS patients resist the Leptin.This research analyzed that there is no obvious relation between IGF-1 and Leptin in PCOS patients' blood serum.So it can be speculated that they may have their respective independent regulation system with reference to PCOS causing mechanism.Or they may have interaction internally.But detail mechanism is still to be discussed.Currently, PCOS causing mechanism has not yet been figured out clearly.But this thesis has found that IGF-1 and Leptin are thick with PCOS symptoms, which can be utilized to remedy hyperinsulinemia and hyperandrogenemia, and reduce the patients' clinical symptoms.To search for an effective brand new treating method,we sincerely hope that monitoring IGF-1 and Lepti can be regarded as indicator with the function of evaluating PCOS severity and post-cured clinical indexing.
